摘要
目的探讨Xpert MTB/RIF检测肺结核及利福平耐药性的应用价值。方法收集200例疑似肺结核患者的痰标本,进行痰涂片抗酸染色显微镜镜检、改良罗氏固体培养、比例法药敏试验和Xpert MTB/RIF检测,分析Xpert MTB/RIF检测结核分枝杆菌及其利福平耐药性的敏感度和特异度,与"金标准"进行一致性检验。结果以改良罗氏固体培养为"金标准",Xpert MTB/RIF检测痰标本MTB的敏感度和特异度分别为88.30%和87.74%;以痰涂片抗酸染色法为"金标准",Xpert MTB/RIF检测痰标本MTB的敏感度和特异度分别为88.18%和100.00%;以比例法药敏试验为"金标准",Xpert MTB/RIF检测利福平耐药的敏感度和特异度分别为83.33%和100.00%;Xpert MTB/RIF法和改良罗氏固体培养、痰涂片抗酸染色法、比例法药敏试验均具有较好的一致性(K值分别为0.76、0.87、0.77)。结论 Xpert MTB/RIF在肺结核快速诊断中具有较高的诊断效能,在我国结核病实验室中具有较好的应用前景。
Objective To evaluate the value of clinical application of Xpert MTB / RIF test on detecting pulmonary tuberculosis and rifampin resistance. Methods 200 sputa samples were collected form suspected patients with pulmonary tuberculosis,each sputum was detected by sputum smear microscopy,modified Roche solid cultivation methods,proportion method drug susceptibility test and Xpert MTB / RIF test. Analyzed the sensitivity and specificity of Xpert MTB / RIF test in detecting pulmonary tuberculosis and rifampin resistance. Consistency with golden standard was checked. Results Regarding modified Roche solid cultivation as golden standard,the sensitivity and specificity of Xpert MTB / RIF test in detecting tuberculosis are 88. 30% and87. 74%,respectively; regarding sputum smear anti-acid staining method as golden standard,the sensitivity and specificity of Xpert MTB / RIF test in detecting tuberculosis are 88. 18% and 100. 00%; regarding the results of traditional proportion method drug susceptibility test as golden standard,the sensitivity and specificity of Xpert MTB / RIF test in detecting rifampin resistance are 83. 33% and 100. 00%; Xpert MTB / RIF and modified Roche solid cultivation methods,sputum smear anti-acid staining method,traditional proportion method drug susceptibility test( K = 0. 76,0. 87,0. 77,respectively) had good agreements.Conclusion The role of Xpert MTB / RIF test for rapid diagnosis of pulmonary tuberculosis is efficient,and it has the potential to be widely used in tuberculosis laboratories in China.
