摘要
目的:以心率(HR)、心指数(CI)、体循环阻力(SVR)作为效应指标,明确右美托咪啶(Dex)用于SICU镇静时年龄和血流动力学效应的关系。方法:选择2014年3月~7月间在我院SICU接受普胸或者普外科手术后需要短期镇静患者38例,各年龄段分布相对均匀。在病人术后Ramsay评分≤3分时给予右旋美托咪啶6.0μg/kg/h,连续静脉输注10 min后停药,应用脉搏指示连续心输出量监测技术(PICCO)记录用药前及用药后3 min、5min、8min、10min、15min、20 min、30min、45min、60min、90min、120min的11个时间点的HR、CI和SVR。结果:HR、CI和SVR的EMAX随着年龄的增加而增大,可以通过数学模型表示:E=[-exp(E_(max)+delE_(max)·LOG(AGE/50))]·C^r/(C^r+C_(50)~r)(P<0.05)。结论:右美托咪啶用于SICU镇静时,患者HR、CI和SVR的EMAX呈年龄依赖性变化。
Objective:To clarify the correlation between hemodynamic effects and ages when Dexmedetomidine(Dex) used in SICU patients,by taking hart rate(HR),cardiac indexes(CI),and systemic circulation resistance(SVR) as the indexes.Methods:A total of38 patients,who needed short term sedation after chest or general surgery admitted to SICU of Renji Affiliated Hospital of School of Medicine of Shanghai Jiaotong University from March to July 2014,were selected.The distribution of each age group was relatively uniform.All the patients were treated with continuous intravenous infusion of dexmedetomidine(6.0 μg/kg/h) for 10 minutes when their Ramsay scores were ≤ 3 points after operation.Then HR,CI and SVR at 3 min,5 min,8 min,10 min,15 min,20 min,30 min,45 min,60 min,90 min,120 min before and after treatment were measured by pulse index continuous cardiac output(PICCO) method.Results:The EMAX of HR,CI and SVR was enlarged along with the increasing of age,which could be showed by mathematical model:E=[-exp(E_(max)+delE_(max)·LOG(AGE/50))]·C^r/(C^r+C_(50)~r)(P〈0.05).Conclusion:The Emax of HR,CI and SVR of SICU patients Presents age dependent changes when dexmedetomidine is used for sedation.
