摘要
目的观察右美托咪啶对甲状腺手术患者丙泊酚-雷米芬太尼麻醉的影响。方法甲状腺腺瘤切除术患者40例,应用靶控输注(TCI)丙泊酚-雷米芬太尼进行麻醉诱导与维持;随机均分为两组,A组在手术结束前45min左右输注右美托咪啶1μg/kg,B组输注生理盐水作为对照。观察围麻醉期及不同脑电双频指数(BIS)值(60、70、80)的丙泊酚TCI靶浓度及平均动脉压、心率,并记录TCI结束至睁眼时的苏醒时间。结果 A组术毕至拔管各时间点的丙泊酚效应室靶浓度明显低于B组(P<0.01)。A组苏醒时间(21.0±6.6)min,明显长于B组的(15.5±3.3)min(P<0.05)。结论甲状腺腺瘤手术患者行丙泊酚-雷米芬太尼TCI麻醉时,输注右美托咪啶1μg/kg能使丙泊酚的效应室靶浓度降低,但苏醒时间略有推迟。
Objective To investigate the effects of dexmedetomidine on anesthesia with target controlled infusion(TCI) of propofol and remifentanyl in patients undergoing thyroidectomy.MethodsForty adult patients scheduled for selective thyroidectomy were anaethetized with propofol and remifentanyl TCI,who were equally randomized into two groups of A(infused dexmedetomidine 1 μg/kg in 50 ml normal saline at 45 min before the end of operation)and group B(infused normal saline 50 ml as the control).The mean arterial pressure(MAP) and heart rate(HR) were monitored during anesthesia and at different BIS values of 60,70 and 80.The propofol target concentration and the recovery time from anesthesia were recorded as well.Results The effective-site target concentrations of propofol during recovery period were significantly lower in group A than those in group B(P0.01).The recovery time of group A was(21.0±6.6) min,which was longer than(15.5±3.3) min in group B(P0.05).Conclution Dexmedetomidine 1 μg/kg infused during TCI of propofol and remifentanyl can reduce the effective-site target concentrations of propofol,but prolong the recovery time from anesthesia in patients undergoing thyroidectomy.
