摘要
目的:建立海水浸泡兔开放性脊髓损伤模型,观察高压氧联合甲基强的松龙对海水浸泡兔脊髓损伤的影响。方法80只日本大耳家兔,体质量2.5~3㎏,雌雄不拘,采用改良Allen's打击器制造T10段兔脊髓损伤模型,并给予海水浸泡处理,按数字表法随机分为4组,每组20只,分别为对照组、高压氧( HBO)组、甲基强的松龙( MP)组以及两者联合干预的联合组。分别于致伤后6、12、24、48 h 4个时间点行神经功能评分( Tarlov法)、免疫组织化学染色检测凋亡相关基因Bax、B淋巴细胞瘤-2基因(Bcl-2)及神经丝蛋白(NF200)表达、原位末端标记(TUNEL)法检测脊髓神经细胞凋亡情况。结果HBO组及MP组和联合组的Tarlov评分及病理学指标均优于对照组,差异有统计学意义(P<0.05);联合组(48 h Tarlov评分为3.92±0.37,Bax、Bol-2和NF200的吸光度值分别为73.1±5.6、149.1±5.5、122.6±4.3,凋亡细胞数为2.1±1.1)的相关指标则优于其他治疗组(48 h HBO组Tarlov评分为2.98±0.55,Bax、Bcl-2和NF200的吸光度值分别为97.5±2.1、116.5±2.2、80.8±3.3,凋亡细胞数为5.5±1.2;MP组Tarlov评分为2.95±0.52,Bax、Bol-2、和NF200的吸光度值分别为98.5±3.1、117.1±3.6、93.2±4.4,凋亡细胞数为5.3±1.3),差异具有统计学意义(P<0.05)。结论单独应用HBO或MP的干预措施通过抑制Bax,促进Bcl-2及NF200的表达,减少脊髓神经细胞凋亡,促进脊髓损伤修复,对海水浸泡脊髓损伤的神经细胞起保护作用,而两者联合应用能发挥协同、增强作用。
Objective To establish an animal model of open spinal cord injury associated with seawater immersion, and then to observe the effects of hyperbaric oxygen (HBO), methylprednisolone (MP) and MP +HBO on the spinal cord injury associated with seawater immersion , and finally to evaluate the therapeutic effects of the 3 intervention methods and analyze the advantages and disadvantages of the 3 treatment methods.Methods Eighty Japanese white rabbits with a weight range from 2.5kg to 3kg were used in the study.The animal model of the 10th thoracic spinal cord injury was developed by using the modified Allen 's method, and then they were exposed to artificial seawater .The animals were randomly divided into 4 groups, the control group, the HBO group, the MP group, and the MP +HBO group, each consisting of 20 animals. Respectively at 4 different time points after injury , i.e.at hours 6, 12, 24 and 48, neurological function scores were evaluated by using the Tarlov method , and the expression levels of Bax , Bcl-2 and NF200 were detected by immunohistochemistry , and apoptosis of neurons in the spinal cord tissue was evaluated by the TUNEL method.Results Both the Tarlov scores and histopathological indexes of the MP +HBO group and the MP group were superior to those of the blank control group , and statistical differences could be noted , when comparison were made between them (P〈0.05).The detected data of the MP +HBO group were superior to the other treatment groups , and statistical differences could be noted , when comparison were made between them (P〈0.05).Conclusions Separate application of HBO or MP could reduce apoptosis of spinal cord cells and promote recovery of the injured spinal cord through the inhibition of Bax and reduction of the expression levels of Bcl-2 and NF200, thus alleviating injury to neural cells of the spinal cord .Nevertheless, the combined application of HBO and MP would result in synergistic and enhancing effects on the damaged tissue.
