不同代谢综合征诊断标准在高血压人群中的比较 被引量:7

Predictive value of the different definitions of metabolic syndrome among hypertension population

摘要 目的比较国际糖尿病联盟(IDF)以及中华医学会糖尿病学分会(CDS)代谢综合征(MS)诊断标准在高血压人群中应用的异同。方法收集7 485例高血压[≥140 mm Hg和(或)90 mm Hg]患者流行病学资料,分别应用IDF和CDS标准,比较MS的检出率及诊断的一致性,并分析不同性别、年龄及代谢异常构成的分布特点。结果 IDF和CDS标准MS检出率分别为51.7%和42.5%,以IDF标准诊断的MS检出率较高(P<0.001)。与CDS标准相反,IDF标准女性MS检出率高于男性(P<0.001);不同标准各年龄组MS检出率变化趋势相似,随着年龄增高而增高。高血压合并2种代谢异常IDF标准检出率较高(P<0.001)。结论 MS在高血压人群中普遍存在;IDF标准在高血压人群中检出率较高,IDF更有利于早期控制高血压人群的多种危险因素。 Objective The metabolic syndrome(MS) is a common metabolic disorder that results from the increasing risk of developing cardiovascular disease and/or diabetes mellitus. We set up to evaluate the International Diabetes Federation(IDF) and Chinese Diabetes Society(CDS) definitions in diagnosing MS in hypertension patients. Methods This cross-sectional study included 7 485 adults. Anthropometry and blood pressure were measured and bio-specimens were collected. The prevalence of MS in hypertension patients and the unity of the two definitions were studied. Results The overall prevalence of MS was 51.7% and 42.5% according to IDF and CDS definitions respectively. The diagnostic rate of IDF definition was higher(P〈0.001). The prevalence of the MS was higher in males(46.3%) than in women(40.0%) using CDS definition, but this sex difference was reversed when using the IDF(43.9% for males vs. 56.7% for females). Different definitions showed the same increased prevalence of MS with age. In the hypertension patients with two metabolic disorders, the diagnostic rate of the definition of IDF was higher(P〈0.001). Conclusion The MS is highly prevalent among hypertension population. Compared with CDS definition, the diagnostic rate of MS by IDF definition was higher in hypertension patients, addressing more aggressively control global risk factors.
