摘要
目的探讨异丙酚复合瑞芬太尼麻醉对脑膜瘤手术患者血流动力学及SI00B的影响。方法从本院2013年12月至2014年12月收治的脑膜瘤患者中随机选择63例进行研究,随机分为对照组(31例)和观察组(32例),均实施手术治疗,并分别给予异丙酚麻醉和异丙酚复合瑞芬太尼麻醉。于麻醉后1 h(T1)、2 h(T2)、缝合硬膜时(T3)和手术结束时(T4)对两组患者进行采血,检测其血流动力学变化情况,包括血氧饱和度(Sp O2)、心率(HR)、平均动脉压(MAP),并对患者的血清SI00B蛋白含量进行检测。结果观察组患者的血流动力学各项指标在不同时间点均未发生明显的改变(P>0.05);但与对照组各时间点比较,差异均有统计学意义(P<0.05),表明观察组维持了较为稳定的血流动力学状态。麻醉后,两组患者的SI00B蛋白水平均明显上升(P<0.05);两组间比较,差异均有统计学意义(P<0.05)。结论对脑膜瘤手术患者实施异丙酚复合瑞芬太尼麻醉可以维持稳定的血流动力学状态,并可对患者进行脑保护。
Objective To investigate the influence of propofol and remifentanil anesthesia on blood flow dynamics and SI00B in pa-tients with meningioma operation. Methods Totally 63 patients of meningioma were randomly selected from our hospital from Decem-ber 2013 to December 2014 in the study,and they were randomly divided into control group（31 cases）and observation group（32 ca-ses）. Patients in the two groups were admitted with propofol,and propofol combined with remifentanil anesthesia,respectively. Blood sampling were performed in different time（ T1 ,1 hour;T2 ,2 hours;T3 ,dural suture;T4 ,at the end of surgery） for patients of the two groups,for detecting hemodynamic changes,including oxygen saturation（SpO2）,heart rate（HR）,mean arterial pressure（MAP）,and the patients serum SI00B protein content were detected. Results Hemodynamics indexes of patients in the observation group at differ-ent time points were not changed obviously（P〉0. 05）,but when compared with the control group,there were statistical differences （P〈0. 05）,showed that patients in the observation group could maintain hemodynamic state,and relatively stable after anesthesia. SI00B protein levels in the two groups both raised obviously（P〈0. 05）;and when comparison between the two groups,there were also significant differences（P〈0. 05）. Conclusion The implementation of propofol combined with remifentanil anesthesia on patients with meningioma operation can maintain a stable hemodynamic state,and brain protection of patients.
