MR磁敏感加权成像与CT在颅脑外伤中的诊断价值比较 被引量:8

摘要 目的:探讨磁敏感加权成像(SWI)序列与CT在颅脑外伤血肿中的诊断价值比较。材料与方法:20例颅脑外伤患者,行MRI常规序列、SWI序列及CT平扫检查,分析颅脑外伤MR(SWI)序列与CT检出病变的敏感性。结果:复合型外伤8例,单一性外伤12例,其中包括硬膜外血肿2例,硬膜下血肿3例,脑挫裂伤并血肿7例。SWI序列上硬膜下及硬膜外血肿表现为高或低信号,另可见高低混杂信号,脑内血肿周围常见水肿高信号,脑室、蛛网膜下腔出血呈铸型或线状低信号影。20例外伤患者中合并弥漫性轴索损伤4例,SWI序列可见病灶数为12个,包括水肿伴出血灶8个,单独水肿灶4个,SWI序列呈多发、散在斑点状低信号灶及夹杂片状高信号影;常规CT平扫可见病灶7个,包括水肿伴出血灶2个,单纯水肿灶5个。脑挫裂伤并少量出血7例,SWI序列可见病灶数为11个,包括水肿伴出血灶8个,单独水肿灶2个,SWI序列呈高信号水肿区内斑片或斑点样低信号灶;常规CT平扫可见病灶7个,包括水肿伴出血灶6个,单纯水肿灶1个。MRI(SWI)检出弥漫性轴索损伤及脑部轻微挫裂伤的敏感性优于常规CT(X2=7.1,P<0.05)。结论:颅脑外伤性病变,MRI(SWI)序列较之CT可以更有效提高特别是对弥漫性轴索损伤及小的脑挫裂伤血肿的检出率。可以有效评价颅脑外伤的损伤程度,可以作为颅脑外伤的常规检查序列。 Purpose: To investigate the diagnostic value of magnetic susceptibility weighted imaging (SWI) sequence and CT in brain injury and hematoma. Materials and Methods : 20 cases of traumatic brain injury patients, the MRI routine sequence, SWI sequence and CT scan, analysis of brain injury MR (SWI) sequence and CT detection of the sensitivity of the lesion. Results: 20 cases of trauma patients with diffuse axonal injury in 4 cases, the SWI sequence can see the number of lesions is 12. Conventional CT scan showed that the lesion was 7. MRI (SWI) was superior to conventional CT (x^2 =7. 1, P 〈 0. 05) in the detection of diffuse axonal injury and slight contusion and laceration of brain. Condusion: MRI (SWI) sequence was more effective than CT in brain injury, especially in the detection of diffuse axonal injury and small cerebral contusion and laceration. Can effectively evaluate the degree of brain injury, can be used as a routine examination of brain trauma sequence.
