呼吸科产耐药酶肺炎克雷伯菌感染的危险因素分析 被引量:2

Risk factors for infection with Klebsiella pneumoniae producing drug-resistant enzymes in respiratory department

摘要 目的探讨呼吸科产耐药酶肺炎克雷伯菌感染的分布情况及其危险因素。方法收集福建省立医院呼吸科2009年1月1日至2013年12月31日确诊为医院内感染肺炎克雷伯菌的住院患者资料共121例,分为产耐药酶的病例组41例和未产耐药酶的对照组80例,通过成组t检验、四格表χ^2检验行单因素分析,及进一步多因素Logistic回归分析,分析各因素与产耐药酶肺炎克雷伯菌的关系。结果单因素分析结果显示,年龄、住院时间、调整住院时间、感染前使用抗生素种数、感染前使用第一代头孢、感染前使用第三代头孢均与呼吸科感染产耐药酶肺炎克雷伯菌有关(P〈0.05或P〈0.01);多因素Logistic回归分析发现,年龄大(OR=1.829,P=0.029)、住院时间长(OR=1.590,P=0.025)是产耐药酶肺炎克雷伯菌感染的独立危险因素。结论年龄大、住院时间长的住院患者为产耐药酶肺炎克雷伯菌感染的主要高危人群,应根据此危险因素采取有效措施预防呼吸科产耐药酶肺炎克雷伯菌的感染。 Objective To investigate the distribution situation and risk factors of Klebsiella pneumoniae producing drugresistant enzymes in the respiratory department.Methods Data of 121 patients with confirmed nosocomial infection caused by Klebsiella pneumoniae who admitted in respiratory department of Fujian Provincial Hospital from January 1,2009 to December 31,2013 were collected,and the patients were divided into case group infected by Klebsiella pneumoniae producing drug-resistant enzymes(n = 41)and control group infected by Klebsiella pneumonia that didn't produce drug-resistant enzymes(n = 80).The associations of different factors with infection of Klebsiella pneumoniae producing drug-resistant enzymes were analyzed by single factor analysis with Group t-test and four table Chi-square test first and then the multi-factor Logistic regression analysis.Results Single factor analysis showed that the age,time of hospital stay,time of adjusted hospital stay,the number of used antibiotics,the first-generation cephalosporins use and the third-generation cephalosporins use before infection were significantly associated with infection of Klebsiella pneumoniae producing drug-resistant enzymes in the respiratory department(all P 0.05).Multi-factor Logistic regression analysis showed that older age(OR = 1.829,P =0.029),long hospital stay(OR = 1.590,P = 0.025)were the independent risk factors for infection of Klebsiella pneumoniae producing drug-resistant enzymes.Conclusions The hospitalized patients with older age and long hospital stay are the main high risk population infected by Klebsiella pneumoniae producing drug-resistant enzymes,and effective measures based on these risk factors should be taken to prevent the infection of Klebsiella pneumoniae producing drug-resistant enzymes in the respiratory department.
