摘要
目的:了解南京鼓楼医院集团仪征医院(以下简称"我院")住院医嘱中的不合理用药现象,为医师用药提供参考,提高医疗质量。方法:对2014年我院床药师点评的住院病历1 982份中不合格用药医嘱进行统计、分析。结果:所点评的1 982份住院病历用药医嘱中,不合理用药医嘱有356份,占17.96%。用药医嘱不合理主要表现在围术期抗菌药物选用不当、联合用药不合理、中药注射剂溶剂选用不当、药物的用法和用量不当以及用药书写不规范等。结论:临床药师点评住院现场病历,能够及时发现不合理用药现象,可为医师安全合理用药提供参考。
OBJECTIVE： To investigate the irrational drug use phenomenon in inpatient medical orders from Drum Tower Hospital Group of Nanjing Yizheng Hospital（ hereinafter referred to as ＂our hospital＂）,to provide reference of drug use for physicians so as to improve the medical quality. METHODS： 1 982 inpatient medical records were reviewed by clinical pharmacists of our hospital in 2014,among which the unqualified medical orders were statistical analyzed. RESULTS： Of 1 982 cases,356 medical orders were irrational drug use,accounting for 17. 96%. The irrational situation mainly involved in improper selection of perioperative antibiotics,improper combination of drugs,improper use of solvent for traditional Chinese medicine injection,irrational usage and dosage and non-standard writing of medical orders,etc. CONCLUSIONS： The evaluation of medical orders by clinical pharmacists could find irrational drug use problems in time,and provide reference of rational drug use for the physicians.
