糖尿病合并冠心病患者动态心电图的心率变异性和碎裂QRS波特征分析 被引量:9

Analysis of Heart Rate Variability and Fragmentation Characteristics of QRS Wave in Patients with Coronary Heart Disease and Diabetes with Dynamic Electrocardiogram

摘要 目的分析糖尿病合并冠心病患者应用24 h动态心电图(Holter)检测心率变异性和碎裂QRS波电位(f QRS)特征,了解其诊断心肌缺血和心血管事件中的价值。方法连续选择2014年2月-2015年2月诊断为2型糖尿病和稳定型心绞痛患者共84例(A组),同期选择单纯2型糖尿病患者53例(B组)和单纯稳定型心绞痛患者51例(C组),所有患者均根据相应指南接受治疗,对比分析三组患者的Holter指标差异性。结果 A组患者比B、C组的心率变异指数SDNN显著降低,轻中度减低比例显著高于B、C组,重度减低比例显著低于B、C组,24 h平均心率显著增加,差异均有统计学意义(P〈0.05).A组患者的f QRS发生率显著高于其他两组,差异有统计学意义(P〈0.05).A组患者发生ST-T段平均压低幅度显著大于其他两组,心绞痛发生次数和持续时间显著增加,差异均有统计学意义(P〈0.05)。结论 Holter检测心率变异性和f QRS对预测糖尿病合并冠心病患者心血管事件有一定的应用价值。 Objective To study the characteristics of heart rate variability and QRS wave fragmentation( f QRS) in patients with coronary heart disease and diabetes with 24 h dynamic electrocardiogram( Holter). Methods A total of 84 consecutives diagnosed as coronary heart disease and diabetes from February 2014 to February 2015 as A group,and 53 patients diagnosed as single type 2 diabetes as B group,51 patients diagnosed as single stable angina pectoris as C group,all patients received treatments according to the guidelines,then to compare the differences of heart rate variability and f QRS performances. Results The SDNN value in A group was significantly lower and the mild to moderate rate was higher,severe rate was less than the other two groups( P 〈0. 05). the 24 mean heart rate and f QRS rate were both significantly more than the other two groups( P 〈0. 05). the lowering mean depression of ST- T segment and number,continuous time of angina pectoris in in A group were all significantly higher( P 〈0. 05). Conclusions Heart rate variability and f QRS by Holter could better predict cardiac ischemic events.
