摘要
目的以尸体解剖结果为金标准,评估MR3D-FIESTA序列对颈椎椎间孔外韧带的成像能力。方法分为三部分:(1)取5具正常成人完整颈椎防腐标本,排除颈部畸形、病变或颈椎手术者。选取MR3D-FIESTA序列对标本进行影像学扫描。由放射学家在MR图像上辨认C4-T1椎间孔处韧带结构。(2)标本扫描后,由解剖学家对标本C4-T1椎间孔进行大体及手术显微镜下解剖。(3)以尸体解剖学结果为金标准,评估MR对颈椎椎间孔外韧带的显示能力,计算特异性、敏感性及准确率。结果 C4-T1椎间孔处横孔韧带出现率为50%。放射学家利用MR3D-FIESTA序列扫描图辨别横孔韧带的相关能力如下,特异性91.7%,敏感性35.3%,准确率68.3%。结论 MR3D-FIESTA序列可以提供非常好的分辨率,能够对颈椎椎间孔外韧带进行清晰显示,具有较高的临床应用价值。放射学家在充分掌握颈椎椎间孔外韧带解剖知识的前提下,在3D-FIESTA序列扫描图中认为某颈椎椎间孔存在横孔韧带时,这个颈椎椎间孔确实存在横孔韧带的可能性大约为70%;另一方面,当放射学家认为某椎间孔不存在横孔韧带时,这个颈椎椎间孔仍有大约30%的可能性存在横孔韧带。
Objective To assess imaging capability of MR 3D - FIESTA sequence for extraioraminal liga- ment of cervical vertebra, as the gold standard of autopsy results. Methods Three - part study to： （1）We choosed 5 normal adult complete corrosion specimens of cervical vertebra, eliminating neck deformity, disease or cervical spine surgery. We scaned the specimens by MR 3D - FIESTA sequence. Then radiologists identifued the C4 - T1 intervertebral foramen ligament structure on MR images. （2）After scanning specimens, intervertebral foramen of C4 - T1 were broadly dissected and surgically dissected at microscope by anatomists. （3）With corpse anatomy results as the gold standard, to evaluate the showing ability of extraforaminal ligament of cervical verte- bra for MR, and to calculate the specificity, sensitivity and accuracy. Results The showing rate of horizontal hole ligaments is about 50 % in C4 - T1 intervertebral foramen. Radiologists identified the related capacity of horizontal hole ligaments on 3D - FIESTA sequence as follows： specificity of 91.7 %, sensitivity of 35.3 %, and accuracy of 68.3 %. Conclusion MR 3D - FIESTA sequence could provide a very good resolution, display dis- tinctly extraforaminal ligament of cervical vertebra, and have high clinical value. On the premise of that radiolo- gists fully grasped the anatomical knowledge of the extraforaminal ligament of cervical vertebra , they believed horizontal hole ligaments of the cervical intervertebral foramen were existing on 3D - FIESTA sequence, and the possibility of existing for the horizontal hole ligaments of cervical intervertebral foramen was about 70 % ;When radiologist thought, on the other hand, there was no horizontal hole ligaments in a intervertebral foramen, the horizontal hole ligaments was still exist for the possibilities of 30 % in the cervical intervertebral Ioramen.
