摘要
目的运用三维快速平衡稳态成像(3D-FIESTA)序列显示活体眼运动神经脑池段,同时与先天性眼外肌纤维化综合征(CFEOM)患者进行对照,探讨该序列的临床应用价值。资料与方法20名健康志愿者和3例来自同一家系的CFEOM患者分别行MRI检查,运用多平面重组(MPR)技术进行多维重组,分别在沿神经长轴的层面显示眼运动神经的走行和毗邻结构。结果20名志愿者,40侧动眼神经脑池段全程在斜矢状面、斜横断面及冠状面MR图像上显示清晰,显示率100%(40/40);39侧外展神经脑池段全程在沿神经走行的各个断面均显示清晰,显示率97.5%(39/40);25侧滑车神经脑池段(CN4)于中脑背侧下丘下外方水平出中脑进入环池,显示率62.5%(25/40)。3例(CFEOM组)眼运动神经脑池段均有不同程度发育不良。结论运用3D-FIESTA序列可以清晰显示各组眼运动神经脑池段,能够为眼运动神经发育异常提供直观的形态学依据,辅助临床诊断。
Objective To display the oculomotor cranial nerves by 3D FIESTA and compare with dominant congenital fibrosis of extraocular muscles (CFEOM), and evaluate its clinical value. Materials and Methods MRI was performed in 20 volunteers without strabismus and 3 cases with CFEOM on axial plane using three dimensional fast imaging employing steady state acquisition (3D-FIESTA) sequence without enhancement. Oculomotor nerves were observed in oblique axial, oblique sagittal and coronal planes acquired with MPR. Results 40 oculomotor cranial nerves were demonstrated well in all three planes in 20 normal subjects ( 100% ). 39 abducens (CN6) of 40 nerves (97.5 % ) were displayed in all three planes. 25 trochlear nerves (CN4) of 40 (62.5%) were showed. Different degree hypoplasia of motor nerves, especially CN6, was exhibited in CFEOM. Conclusion All ocular motor nerves can be detected in the subarachnoid cisterns by using 3D FIESTA, 3D FIESTA provides anatomic information for the diagnosis of nerve EOM abnormalities.
