摘要
目的:分析比较不同剂量右美托咪定复合地佐辛在子宫全切术后患者自控静脉镇痛中的临床效果。方法:选取我院行腹腔镜子宫全切术患者120例。分为观察A组、观察B组、观察C组、对照组。观察A组为右美托咪啶0.5μg/kg+地佐辛0.8mg/kg+0.9%氯化钠注射液+托烷司琼8mg;B组为右美托咪啶1μg/kg+地左辛0.8mg/kg+0.9%氯化钠注射液+托烷司琼8mg;C组为右美托咪啶1.5μg/kg+地左辛0.8mg/kg+0.9%氯化钠注射液+托烷司琼8mg;对照组为地佐辛0.8mg/kg+0.9%氯化钠注射液注射液+托烷司琼8mg。观察患者术后1h、6h、12 h、24h、48h的VAS、Ramsay评分、收缩压、舒张压、心率、血液中5-HT、P物质以及儿茶酚胺浓度,48h内不良反应情况。结果:与对照组比较,观察A组、观察B组术后6、12 h VAS对照显著降低,观察A组术后1 h VAS显著增高。与对照组相比,观察A组术后1h及观察B组术后1、6 h和观察C组术后1、6、12、24 h的Ramsay镇静评分增高;与观察A组相比,观察B组术后6h和对照组术后1、6h的Ramsay镇静评分增高;与观察A组相比,观察B组术后1h的Ramsay镇静评分增高。观察A组、B组不良反应发生率明显低于对照组;与观察A组、对照组相比,观察B组血压和心率以及肾上腺素和去甲肾上腺素波动更加平稳(P<0.05),且观察组血浆SP浓度与5-HT明显低于对照组(P<0.05),组间无显著差异(P>0.05)。结论:右美托咪定复合地佐辛在子宫全切术后患者自控静脉镇痛中的临床效果显著,但临床应用时要注意使用合适剂量,减少不良反应。
Objectives：To compare the effects of different doses of dexmedetomidine combined with Zootoxin Kai Fun in the treatment of self-controlled intravenous analgesia of patients receiving hysterectomy.Methods：120 laparoscopic hysterectomy uterus patients in our hospital were selected and divided into observation group A,observation group B,observation group C and control group.Observation group A was treated with 0.5 ug/kg dexmedetomidine ＋ 0.8m/kg dezocine ＋ 0.9% sodium chloride injection ＋ 8mg Tropisetron.Observation group B was treated with 1ug/kg dexmedetomidine ＋ 0.8m/kg dezocine ＋ 0.9% sodium chloride injection ＋ 8mg Tropisetron.Observation group C was treated with 1.5 ug/kg dexmedetomidine ＋ 0.8m/kg dezocine ＋ 0.9%sodium chloride injection ＋ 8mg Tropisetron.Control group was treated with 0.8m/kg dezocine ＋ 0.9% sodium chloride injection ＋ 8mg Tropisetron.VAS,Ramsay score,systolic blood pressure,diastolic blood pressure,heart rate,blood 5-tp substances and catecholamine concentrations 1h,6h,12 h,24h,48 h after the operation were recorded as well as the adverse reactions within 48 hours.Results：Compared with the control group,6,12 h VAS of observed group A and group B were significantly lower.1 h VAS of observed group A was increased significantly.Compared with the control group,1h Ramsay sedation score of observation group A,1,6 h Ramsay sedation score of observed group B and 1,6,12,24 h Ramsay sedation score of observation group C increased;compared with the observation group A,6h Ramsay sedation score of observation group B and 1,6h Ramsay sedation score of controlgroup increased;compared with the observation group A,1h Ramsay sedation score of observation group B increased.Compared with observation group and control group,blood pressure and heart rate,epinephrine and epinephrine of observation group B were more stable and the plasma SP concentration and 5-HT of observation group were significant lower than these of control group（ P〈 0.05）.Conclusions：Dexmedetomidine combine
