摘要
目的观察活血祛瘀中药溻渍联合西药治疗强直性脊柱炎(AS)活动期的临床疗效及安全性。方法将AS活动期患者随机分为西药组41例和中西医结合组45例。西药组给予来氟米特片口服,每次20 mg,每日1次;双氯芬酸钠胶囊口服,每次50 mg,每日2次,1周后按病情减量或停药,必要时临时加量。中西医结合组在西药组治疗的基础上加用活血祛瘀中药在疼痛部位溻渍治疗。两组均治疗3个月。比较两组治疗前后巴氏强直性脊柱炎功能指数(BASFI)、巴氏强直性脊柱炎疾病活动指数(BASDAI)、中医证候积分、红细胞沉降率(ESR)、C反应蛋白(CRP)、生活质量、不良反应,并进行临床疗效评价。结果中西医结合组临床疗效总有效率为93.33%,明显高于西药组的70.73%(P<0.05)。两组治疗后BASDAI、BASFI、ESR、CRP、生活质量评分、中医证候积分均明显改善(P<0.05),且中西医结合组改善情况均优于西药组(P<0.05)。两组不良反应比较差异无统计学意义(P>0.05)。结论活血祛瘀中药溻渍联合西药治疗AS活动期疗效确切,能提高生活质量,且无明显不良反应。
Objective To observe the therapeutic effect and safety of fomentation with promoting blood circulation for removing blood stasis herbs combined with western medicine treating ankylosing spondylitis（AS）in active stage.Methods AS patients in active stage were randomized into western medicine group（41 cases）and Chinese medicine combining with western medicine group（45 cases）.Western medicine group were given leflunomide tablets（orally,20 mg each time,once each day）and diclofenac sodium capsules（orally,50 mg each time,twice each day）.After 1 week,according to disease changes,the medicines were reduced,withdrew or the amount was added temporarily when necessary.Based on western medicine group＇s treatment,Chinese medicine combining with western medicine group added fomentation treatment at painful parts with promoting blood circulation for removing blood stasis herbs.Both groups were treated for 3 months.Bath Andylosing Spondylitis Function Index（BASFI）,Bath Ankylosing Spondylitis Disease Activity Index（BASDAI）,Chinese medicine syndrome scores,Erythrocyte Sedimentation rate（ESR）,C-Reactive Protein（CRP）,quality of life（QOL）score and side effect were compared between both groups before and after treatment.And clinical effect was made evaluated.Results Total effective rate in Chinese medicine combining withwestern medicine group was 93.33%,much higher than that in western medicine group（70.73%,P〈0.05）.After treatment,BASDAI,BASFI,ESR,CRP,QOL score and Chinese medicine syndrome scores in both groups were significantly improved（P〈0.05）.The improvement in Chinese medicine combining with western medicine group was superior to that in western medicine group（P〈0.05）.The difference of side effect in both groups was not significant（P〉0.05）.Conclusion Fomentation with promoting blood circulation for removing blood stasis herbs combined with western medicine treating AS in active stage was effective,and it could improve quality of life and had no obvious side effect.
