摘要
目的探讨标准化粉尘螨滴剂特异性免疫治疗小儿过敏性哮喘的疗效及其对肺功能的影响。方法收集浙江省永康市第一人民医院2012年1月至2015年1月小儿变应原皮肤测试阳性患者130例,按照随机分配原则分为两组,观察组(65例)和对照组(65例)。观察组采取常规抗过敏疗法,在此基础上以粉尘螨滴剂舌下含服,对照组对患儿仅采取常规抗过敏治疗。观察并比较观察组和对照组的疗效及患儿肺功能指标变化情况。结果患儿在咳嗽、肺部啰音、呼吸困难等方面,观察组比对照组改善时间均明显缩短,差异均具有统计学意义(t值分别为7.389,7.265,7.030,均P<0.05)。治疗前对两组患儿的临床症状进行评分,第1秒内呼吸气体的容积(FEV1),测定呼气峰流速(PEF)及FEV1/FVC无明显差异(t值分别为0.591,-1.288,0.187,均P>0.05)。治疗后观察组患儿肺功能指标FEV1、PEF、FEV1/FVC与对照组相比,改善效果显著,差异有统计学意义(t值分别为14.010、11.829、7.211,均P<0.05)。结论标准化粉尘螨滴剂特异性免疫治疗小儿过敏性哮喘能有效提高疗效,对肺功能改善效果良好,对患儿预后较好。
Objective To discuss the efficacy of specific immune treatment of standardized dust mites drops on pediatric allergic asthma and its effect on pulmonary function. Methods Totally 130 cases whose pediatric allergen skin tests were positive were selected from January 2012 to January 2015 in First People＇ s Hospital of Yongkang. They were randomized divided into two groups, observation group （65 cases） and control group （65 cases ）. The observation group took regular allergy therapy and dust mites drops under tongue, while the control group were only provided with routine allergy treatment. The curative effects and pulmonary function indexes changes of two groups were observed and compared. Results The observation group was improved more significantly than the control group in cough, lung tale and dyspnea, and the differences were statistically significant （ t value was 7. 389, 7. 265 and 7. 030, respectively, all P 〈 0.05 ）. Clinical symptoms before treatment of two groups were scored, and there were no significant differences in FEV1, PEF and FEV1/FVC between them （t value was 0. 591, - 1. 288 and 0. 187, respectively, all P 〉 0.05 ）. After treatment the improvement of the observation group was more remarkable than the control group and the differences in pulmonary function indexes （FEV1, PEF and FEV1/FVC） were statistically significant （ t value was 14. 010, 11. 829 and 7.211, respectively, all P 〈 0.05 ）. Conclusion The specific immune therapy of standardized dust mites drops for pediatric allergic asthma can effectively improve curative effect, and the pulmonary function is improved better with good prognosis.
