摘要
目的探讨EV71脑炎患儿血清和脑脊液中白细胞介素-1β(interleukin-1β,IL-1β)和IL-1rα水平的变化及其临床意义。方法根据临床表现将患儿分为普通病例组、重症病例组(再分为重症病例重型组和重症病例危重型组)。并设立对照组。应用双抗体夹心ELISA法分别检测各组患儿不同时期血清、脑脊液中IL-1β和IL-1rα的水平。结果重症病例组血清和CSF中IL-1β、IL-1ra水平均高于普通病例组和对照组,差异有统计学意义(P<0.05或P<0.01);重症病例危重型组血清和CSF中IL-1β、IL-1ra水平均高于重症病例重型组和对照组,差异有统计学意义(P<0.05或P<0.01);重症病例重型组CSF中IL-1ra的水平高于对照组,差异有统计学意义(P<0.05);重症病例组急性期血清和CSF中IL-1β、IL-1ra含量均高于恢复期和对照组,差异有统计学意义(P<0.05或P<0.01)。结论 IL-1β和IL-1rα可能参与EV71脑炎的发病机制,对EV71脑炎的病情严重性评估及预后判断有一定意义。
Objective To explore the alteration and clinical significance of serurn and eerebrospinal fluid (CSF) interleukin- 1β (IL- 1β) and interleukin - irα ( IL - 1ra) in children with EV71 encephalitis. Methods According to clinical manifesta- tions, children were divided into ordinary case group and severe case group. The later were further subdivided into severe type group of severe cases and critical type group of severe cases. Meanwhile, a control group was set up. Serum and CSF concentra- tions of IL - 1β and IL- lra in each group in different phases were detected by double antibody sandwich ELISA method. Resuits Serum and CSF levels of IL- 1β and IL-1ra in the severe case group were significantly higher than those in the ordinary case group and the control group, with statistically significant differences (P 〈 0.05 or P 〈 0.01 ). Serum and CSF levels of IL- 1β and IL-1ra in the critical type group of severe cases were elevated compared with the severe type group of severe cases and the control group, with statistically significant differences(P〈0.05 or P〈0.01). CSF level of IL- lra in the severe type group of severe cases was higher than that in the control group, with statistically significant differences (P 〈 0.05). Serum and CSF levels of IL - 1β and IL -1ra in the severe case group in the acute phase were significantly higher than those in the recovery phase of se- vere case group and the control group, with statistically significant differences( P 〈 0.05 or P 〈 0.01 ). Conclusions IL- 1β and IL-1ra may participate in the pathogenesis of EV71 encephalitis; and hence, detecting the two parameters may be helpful to evaluating the severity and prognosis of EV71 encephalitis
