摘要
目的研究输血相关性急性肺损伤(transfusion-relatedacutelunginjury,TRALI)SD大鼠肺组织细胞间黏附分子(intercellularcelladhesionmolecule,ICAM)一1基因表达和中性粒细胞(polymorphonuclearneutrophil,PMN)数量的变化,旨在探讨ICAM-1和PMN在TRALI发病中的作用。方法60只SD大鼠按随机数字表法分为正常对照组、脂多糖组、TRALI组、阳性对照组各15只。①正常对照组采用假手术处理;②脂多糖组经大鼠腹腔注射脂多糖(2mg/kg,≤1min完毕),2h后静脉输注生理盐水(约1mL/只);(3)TRALI组腹腔注射脂多糖(2mg/kg)2h后静脉输注血浆(约1mL/只);④阳性对照组静脉输注脂多糖(5mg/kg)诱导急性肺损伤。采用Western印迹法检测血浆CAM-1蛋白表达;采用逆转录聚合酶链反应(RT—PCR)检测大鼠肺组织中ICAM-1基因表达量;采用免疫组化法观察肺组织中ICAM一1蛋白表达。结果阳性对照组、TRALI组血浆ICAM-1蛋白表达均显著高于正常对照组、脂多糖组(0.58±0.09、0.41±0.07VS.0.12±0.03、0.21±0.04,均P〈0.01);与正常对照组肺组织中ICAM-1蛋白含量和ICAM-1mRNA表达[(0.16±0.03)和(0.36±0.05)]比较,阳性对照组[(0.33±0.09)和(1.53±0.17)]和TRALI组[(0.27±0.07)和(1.24±0.11)]明显升高(P均〈0.05);TRALI组BALF中细胞总数和PMN百分比[(310±76.6)×10^6/L和(33.5±11.5)%]明显高于正常对照组[(101±63.8)×10^6/L和(9.8±3.5)%,P均〈0.05)];TRALI组组织含水量和MPO活性明显高于正常对照组(P均〈0.05)。结论TRALI大鼠肺组织中ICAM-1蛋白、ICAM-1mRNA的表达升高,肺组织PMN浸润增加,提示ICAM-1参与了PMN与内皮细胞的黏附和聚集,表明ICAM-1和PMN可能在TRALI的炎症反应过程中发挥重要作用。
Objective To study the expression of intercellular cell adhesion molecule - 1 ( ICAM - 1 ) and the number of polymorphonuclear neutrophil (PMN) in rats with transfusion - related acute lung injury,to explore their possible role in TRALI. Methods Sixty Sprague Dawley (SD) rats were randomLy divided into normal control group ( n = 15 ) with sham operation, positive control group (n = 15 ) with ALI induced by intravenous infusion of lipopolysaccharide (LPS) (5 mg/kg), and TRALI group (n = 15 ) treated by intraperitoneal injection of LPS 2 h before the transfusion of human plasma( about 1 mL), LPS control group ( n = 15 ) treated by intraperitoneal injection of LPS 2 h. The reverse transcription - polymerase chain (RT - PCR) was used to detect ICAM - 1 mRNA. Andimmunohistochemical method was used to detect ICAM - I protein expression, Morphological changes in lung was observed under light microscope. Myeloperoxidase (MPO) in lung homogenate ,wet lung weight to dry lung weight ratio(W/D) were observed. Results Compared with normal control group and LPS control group, Expression of ICAM - 1 protein in plasma were increased significantly in positive control group and TRALI group(0.58 ± 0.09,0.41 ± 0.07 vs. 0.12 ± 0.03,0.21 ± 0.04 ) ( all P 〈 0.01 ) ; Compared with normal control group (0.16 ± 0.03 ) and (0.36 ±0.05 ), positive control group(0. 33 ± 0.09 and 1.53 ± 0. 17 ) and TRALI group (0.27 ±0. 07 and 1.24 ± 0.11 ), Expression of ICAM - 1 protein and ICAM - 1 mRNA were increased significantly in the lung ( all P 〈 0.05 ) ; TRALI group [ ( 310 ±76.6 ) × 10^6/L and ( 33.5 ± 11.5 ) % ], the number of cells in BALF and PMN percentage were significantly higher than that of normal control group[ ( 101 ± 63.8 ) × 10^6/L and (9.8 ± 3.5 ) % ] ( all P 〈 0.05 ) ;TRALI group, tissue water content and MPO activity is significantly higher than normal control group ( all P 〈 0.05 ).
