摘要
目的:分析现阶段我国城市社区卫生签约服务存在的主要问题,为建立我国签约服务模式提供参考。方法:采用问卷调查法对哈尔滨市6家社区卫生服务中心主任及居民进行调研。结果:调查的6家社区卫生服务中心,收入构成主要为业务收入和财政补助,运营成本对政府财政补助依存度高;人员培训支出仅占总支出的0.78%,50%的社区未达到公共卫生医师不少于1名的标准,不重视人员培养;居民对签约服务的知晓率、签约率均不足10%,各项服务利用率不足40%,但满意度均在4左右。结论:目前哈尔滨市城市社区卫生签约服务存在着很多问题,如社区专业人员不足,签约服务利用率低等,建议建立合理的制度、构建有效的签约结构、完善签约项目、采用合理的绩效考核促进签约服务。
Objective： The study aims to analyze the main problem of the signing service mode for community health in the current stage, and provide reference for establishing a signing service mode. Methods： Questionnaire survey was used to investigate the directors and residents in six community health service centers. Results：In the investigation of six community health service centers, the main income was constituted of reve- nue （58.34%） and financial aid （41.05%）. The operating costs high depended on financial aid. The personnel training spending accounts for only 0.78% of total spending and half of the community did not meet the standards of public health physician not less than 1. The community health center did not attach importance to staff training. The knowledge rate on signing service of residents were less than 10% and each service utilization was less than 40, although the satisfaction was about 4. Conclusion： At present, Harbing urban community health signing service sys- tem has many problems, such as a lack of community professionals, low rate of utilization of signing service. The suggestion was to set up a rea- sonable system, build effective contract signing structure, improve the actual signing of the project, and adopt the reasonable performance evalua- tion to promote service.
