摘要
近年来,由于抗生素类药物的滥用,抗肿瘤药物和免疫抑制剂的广泛应用等原因,真菌感染的人数越来越多。在这种形势下,研究人员在传统真菌感染诊断方法的基础上提出了G实验、GM实验以及PCR等新的诊断方法。G实验简单快速,灵敏度高,可以检测出多种真菌,但是不能区分菌种,而且存在假阳性和假阴性结果;GM实验耗时短,阳性检出率高,但只能用于侵袭性曲霉病的检测,而且其最佳阳性界值仍存在争议;PCR是目前最快速的方法之一,但由于缺乏标准化操作和判定标准,未被正式应用于临床。本文将上述三种方法做一综述,旨在发现和总结这些方法的优缺点,以便新的诊断方法的研究与发展。
The number of fungal infections has increased in recent years due to the misuse of antibiotics and the extensive use of anticancer drugs and immunosuppressants.In light of this trend,researchers have developed new methods of diagnosing those infections,such as the(1-3)-β-d-glucan(BG)assay,the galactomannan(GM)assay,and PCR,based on conventional methods.The(1-3)-β-d-glucan(BG)assay is simple,fast,sensitive,and can detect a variety of fungi,but it cannot distinguish strains and it can yield false positives and false negatives.The galactomannan(GM)assay takes little time and detects fungi at a high rate,but it can only be used detect invasive aspergillosis and there is disagreement over its optimal detection limits.PCR is one of the fastest methods,but it has not been formally used in clinical practice due to the lack of standardized procedures and standards to assess its results.This article summarizes these three methods by identifying and summarizing their advantages and disadvantages in order to facilitate research and development of new methods of diagnosing fungal infections.
