摘要
目的探讨胰岛素抵抗指数诊断妊娠期糖尿病的临床价值。方法根据葡萄糖代谢情况将240例孕妇分为6组,即孕早期妊娠期糖尿病组、孕早期健康组、孕中期妊娠期糖尿病组、孕中期健康组、孕晚期妊娠期糖尿病组、孕晚期健康组,每组40例,对比各组胰岛素抵抗指数、胰岛素β细胞功能指数、血糖曲线下面积、胰岛素曲线下面积。结果孕早、中、晚期妊娠期糖尿病患者胰岛素抵抗指数、血糖曲线下面积、胰岛素曲线下面积明显大于同期健康孕妇(P<0.05)。妊娠早、中、晚期妊娠期糖尿病患者的胰岛素β细胞功能指数与同期健康孕妇比较,差异无统计学意义(P>0.05)。结论在妊娠早期监测胰岛素抵抗指数、血糖曲线下面积、胰岛素曲线下面积等指标能够有效预测孕妇糖尿病的发生,具有较高的临床应用价值。
Objective To explore the clinical value of insulin resistance index in the diagnosis of gestational diabetes mellitus（ GDM）. Methods A total of 240 pregnant women were divided into 6 groups according to their data of glucose metabolism,including the first trimester GDM group,the first trimester healthy group,the second trimester GDM group,the second trimester healthy group,the third trimester GDM group and the third trimester healthy group,with 40 cases in each group. The insulin resistance index,insulin β cell function index,area under the curve（ AUC） of blood glucose and AUC of insulin were assessed in each group. Results The insulin resistance indices,AUCs of blood glucose and AUCs of insulin of patients with GMD in the first,second and third trimester were significantly more than those of the healthy pregnant women in the same trimester（ P 〈 0. 05）. No significant difference was found in the insulin β cell function indices between patients with GMD in the first,second and third trimester and the healthy pregnant women in the same trimester（ P 〉 0. 05）. Conclusion In the first trimester,the indices,including insulin resistance index,AUC of blood glucose and AUC of insulin,can effectively predict the occurrence of diabetes mellitus among pregnant women,and have high value of clinical application.
