未动员的无关供者其外周血干细胞采集效果多因素分析 被引量:1

Multivariate analysis of the effect of peripheral blood stem cell collection in unrelated donors without mobilization

摘要 目的探讨未作动员的无关供者其外周血造血干细胞(PBSC)采集效果及其影响因素。方法收集2017年1月-2019年4月134例无关供者在本院未经粒细胞集落刺激因子(G-CSF)动员就用血细胞分离机采集外周血干细胞,分析比较男女2组采集前血常规指标及采集物指标的差异,并分别与采集前白细胞计数、血红蛋白(Hb)、红细胞比容(Hct)、血小板计数(Plt)、单个核细胞(MNC)数、采集时循环血量等因素进行相关性分析。结果所有采集者均采集成功,其外周血中是存在一定数量的MNC、CD34^+细胞。1)男性组采集前Hct、WBC、Hb、MNC数(46.34±2.22、6.71±1.30、159.97±8.71、2.71±0.47)均高于女性组(39.62±2.18、5.67±0.92、134.78±7.80、2.40±0.35),采集物中MNC%及MNC数(46.61±14.51、0.449±0.01)低于女性组(55.18±20.44、0.461±0.02)(P<0.05)。2)男性组采集物MNC数(0.449±0.01)与采前WBC指标(6.71±1.30)成正相关、与循环总容量(5169.02±444.88)成负相关;女性组采集物MNC数(0.461±0.02)与采前WBC、Plt、MNC数(5.67±0.92、249.22±40.19、2.40±0.35)成正相关。3)女性组采集物CD34^+细胞(0.856±0.05)与采集者BMI、采前Plt成负相关(21.21±2.26、249.22±40.19)。4)女性组采集物容量(42.60±2.32)与采前WBC、Hct、Hb、Plt、MNC数指标(5.67±0.92、39.62±2.18、134.78±7.80、249.22±40.19、2.40±0.35)成负相关。结论未作动员的无关供者其外周血中是存在一定数量的MNC、CD34^+细胞,并能采集到满足临床CAR-T细胞疗法所需要的MNC阈值;供者因性别不同其PBSC采集效果不相同;男女两组PBSC采集效果与采集前血常规指标以及采集过程中循环血量等因素均有关联,但相关性各不相同。 Objective To investigate the factors influencing the efficiency of collection of the peripheral blood stem cells(PBSC)in unrelated donors without mobilization.Methods 134 unrelated donors between January 2017 and April 2019,whose PBSC were collected using a blood cell separator in our hospital without mobilization by granulocyte colony stimulating factor(G-CSF),were enrolled in this study.The differences of indicators of blood routine and collecttions between the male and female group were analyzed and compared.We also analyzed the influencing factors,such as the white blood cell(WBC)count,hemoglobin(Hb),hematocrit(Hct),platelet(Plt)count,monocular nuclear cell(MNC)count and circulating blood volume,pre-and during collection process between each groups.Results Peripheral blood were collected successfully from all 134 cases.In addition,we were able to collect a certain number of MNC,CD34^+cells.①The number of Hct,WBC counts,Hb and mononuclear cells(MNC)in male group was higher than that in group female,while the number of MNC%and MNC in the collection was lower than that in female group(P<0.05);②The number of MNC collected by the male group was positively correlated with the pre-harvest WBC index and negatively correlated with the total circulating capacity.The number of MNC collected in the female group was positively correlated with the number of pre-harvest WBC,Plt,and MNC.The CD34^+cells of collection in female group were negatively correlated with BMI of collector and pre-harvest Plt counts.③The collection capacity of female group was negatively correlated with the number of WBC,Hct,Hb,Plt and MNC before harvest.④The collection volume of female group was negatively correlated with the pre-harvest WBC,Hct,Hb,Plt,and MNC indicators.Conclusion Unrelated donors who are not mobilized have a certain number of MNC and CD34^+cells in their peripheral blood,which can collect the MNC threshold required to meet clinical CAR-T cell therapy.The donor’s PBSC collection effect is different due to gender.The PBSC c
