摘要
目的评价血栓弹力图(TEG)试验在体外循环心血管术后指导输血的应用价值。方法回顾性分析体外循环心血管术后309例TEG试验结果。结果术后TEG试验结果,有43.36%(134/309)发生凝血功能下降异常,原因分别为血小板功能减低(15.53%)、肝素残留(16.50%)、纤维蛋白原缺乏(11.97%)、凝血因子缺乏(4.56%),没有同时出现代表3种凝血功能下降的凝血因子缺乏、纤维蛋白原缺乏和血小板功能下降的情况,男女不同性别组间没有差异,术后年龄>50岁组的凝血因子相对≤50岁组缺乏。结论 TEG试验对心血管体外循环术后异常出血原因及指导合理使用血制品具有重要价值。
Objective To analyze the value of thromboelastography(TEG)in guiding blood transfusion after cardiovascular surgery with cardiopulmonary bypass.Methods The results of TEG test in 309 patients after cardiovascular surgery with extracorporeal circulation were analyzed retrospectively.Results According to the results of postoperative TEG test,abnormal coagulation function occurred in 43.36%(134/309).The causes were as following:thrombocytopenia(15.53%),heparin residue(16.50%),fibrinogen deficiency(11.97%)and coagulation factor deficiency(4.56%).There was no simultaneous deficiency of coagulation factors,fibrinogen and platelet function,and there was no difference between male and female groups.The coagulation factor in the group of postoperative age>50 years was less than that in the group of≤50 years old.Conclusion TEG test is of great value in guiding the rational use of blood products and diagnosing the causes of abnormal bleeding after cardiovascular cardiopulmonary bypass.
