枸橼酸咖啡因治疗早产儿原发性呼吸暂停临床观察 被引量:16

摘要 目的 探讨呼吸中枢兴奋药物枸橼酸咖啡因在改善早产儿原发性呼吸暂停(AOP)方面的临床疗效与安全性。方法 将2013年10月-2014年12月温州市中心医院NICU收治的120例诊断有原发性呼吸暂停的早产儿入选试验,按治疗方法 分为治疗组(n=60)和对照组(n=60),在常规综合治疗的基础上,治疗组应用呼吸兴奋药物枸橼酸咖啡因治疗,负荷量20 mg/kg,24 h后给予当日维持量5 mg/(kg·d)每天1次静脉注射,连用5 d,对照组则予呼吸兴奋药物氨茶碱治疗,负荷量5 mg/kg,12 h后给予当日维持量2 mg/kg,每8小时1次静脉注射,连用5 d;回顾分析两组患儿的临床资料,比较两组治疗的有效率及不良反应发生率。结果 两组患儿用药后5 d内疗效比较,治疗组总有效率为88.3%,而对照组有效率为61.7%,治疗组的临床效果明显优于对照组,差异有统计学意义(P〈0.05)。治疗组发生喂养不耐受4例、胃排空延迟3例、心率增快(〉160次/min)1例、高血糖3例、电解质紊乱2例,对照组发生喂养不耐受12例、胃排空延迟11例、心率增快(〉160次/min)8例、高血糖10例、电解质紊乱9例;枸橼酸咖啡因的各种不良反应的发生率比氨茶碱组偏低,差异有统计学意义(P〈0.05)。结论 呼吸中枢兴奋药物枸橼酸咖啡因在改善早产儿原发性呼吸暂停方面优于氨茶碱组,而各种不良反应发生率低于氨茶碱组,可作为治疗早产儿呼吸暂停的首选药物。 Objective To investigate and analyze the clinical efficacy and safety of respiratory center stimulant drug caffeine citrate in improving primary apnea of prematurity(AOP).Methods A total of 120 cases of premature children with primary apnea who were diagnosed and treated in NICU,Wenzhou Central Hospital from October 2013 to December2014,were chosen' into trials and divided into two groups according to treatment,each with 60 cases in the treatment group and the control group.On the basis of routine comprehensive treatment,the treatment group received 1 time of intravenous injection of respiratory stimulant drug caffeine citrate every day for 5 days,with load quantity of 20 mg/kg,and after 24 hours with the day maintenance quantity of 5 mg/(kg·d).The control group received 1 time of intravenous injection of respiratory stimulant drug theophylline every 8 hours for 5 days,with load quantity of 5 mg/kg,and after 12 hours with the day maintenance quantity of 2 mg/kg.The clinical data of the children in the two groups were reviewed and analyzed,and the effective rate and incidence of side effects of the children in the two groups were compared.Results The treatment efficacy of children in the two groups within five days after treatment was compared,and the total effective rate of the treatment group was 88.3%,while the total effective rate of the control group was 61.7%.The clinical effect of the treatment group was better than that of the control group,and the difference was statistically significant(P〈0.05).There were four cases of children with feeding intolerance,three cases of children with delayed gastric emptying,one case of child with increased heart rate( 〉160 beats/min),three cases of children with high blood sugar and two cases of children with electrolyte imbalance in the treatment group,while there were 12 cases of children with feeding intolerance,11 cases of children with delayed gastric emptying,eight cases of children with increased heart rate(〉160beats/min),10 cases of chi
