摘要
目的:评价瞬时弹性成像技术(Fibro Touch)诊断非酒精性脂肪性肝病(NAFLD)的临床应用价值。方法:选取经腹部超声确诊的轻、中、重度NAFLD患者103例及健康体检者(对照组)32例,均计算BMI,检测肝功能、血脂、血糖,同时应用Fibro Touch测量受控衰减参数(CAP)值。比较各组患者CAP值的差异,分析CAP值与肝脂肪变性程度、生化学指标的相关性,并用受试者工作特征(ROC)曲线分析Fibro Touch诊断NAFLD的准确性。结果:各组人员CAP值:对照组(242.89±6.91)db/m,轻度NAFLD组:(246.90±6.49)db/m,中度NAFLD组:(277.71±8.43)db/m,重度NAFLD组:(309.74±11.46)db/m。组间比较除正常组与轻度NAFLD组外,其余均有显著性意义(<0.05);CAP值与肝脂肪变性程度呈正相关,Spearman=0.819;CAP值与BMI、TG、TC、GLU呈正相关,Spearman分别为0.528、0.420、0.327、0.258;Fibro Touch诊断轻、中、重度NAFLD的ROC曲线下面积分别为0.789(0.683,0.894)、0.722(0.588,0.855)、0.739(0.617,0.861),诊断界值分别是247.4、256.7、293.8 db/m。结论:Fibro Touch对NAFLD有较高的诊断价值,可作为NAFLD诊断和随访的依据。
Objective:To evaluate the clinical application value of Fibro Touch in the diagnosis of nonalcoholic fatty liver disease(NAFLD). Methods: One hundred and three patients with mild、moderate and severe NAFLD confirmed by abdominal ultrasound were selected.All patients and 32 cases of healthy control group were performed BMI calculation, liver function blood lipids and blood glucose detection,and Controlled Attenuation Parameter(CAP)values were measured by using Fibro Touch. The difference of CAP values was compared between the groups, and the correlation between CAP value and liver steatosis degree or the biochemical index were analyzed, and the receiver-operating characteristic(ROC) curve was used to analyze the accuracy of Fibro Touch in NAFLD diagnosis. Results: CAP value of each groups were 242.89±6.91db/m in healthy control group, 246.90±6.49db/m in mild NAFLD groups, 277.71±8.43db/m in moderate NAFLD groups and309.74±11.46db/m in severe NAFLD groups. The differences between groups were statistically significant(P〈0.05) except for control group and mild NAFLD group. There was positively correlation between CAP value and liver steatosis degree, Spearman =0.819. There was also positively correlation between CAP value and BMI, TG, TC and GLU, Spearman =0.528, 0.420, 0.327, 0.258. Areas under the ROC curve(AUC) of Fibro Touch in mild, moderate and severe NAFLD diagnosis were 0.798(0.683, 0.894), 0.722(0.588, 0.855) and 0.739(0.617,0.861), and the best cutoff value respectively were 247.4, 256.7 and 293.8db/m. Conclusion:Fibro Touch has a high value in NAFLD diagnostic and could be used as the basis in NAFLD diagnosis and follow-up.
