1型糖尿病儿童25羟维生素D状况及其对骨密度以及骨生化指标的影响 被引量:3

The status of vitamin D in children with type 1 diabetes mellitus( T1DM) and its influence on biochemical and densitometric parameters and the relation to diabetic nephropathy

摘要 目的:探讨1型糖尿病儿童血清25羟维生素 D 状况及其对骨生化和骨密度的影响以及与糖尿病肾病的相关性。方法:42例8~14岁的1型糖尿病患儿纳入研究。分别检测血清25羟维生素 D 以及其他与骨健康相关的生化指标,采用定量骨密度超声仪测定桡骨的骨密度。45例健康儿童检测血清25羟维生素 D。结果:27例(64.3%)患儿25羟维生素 D 浓度﹤75 nmol/ L,其中13例﹤50 nmol/ L,2例﹤25 nmol/ L。1型糖尿病儿童血清维生素 D 显著低于健康儿童。维生素 D 缺乏患儿其血清镁离子以及骨密度显著低于维生素 D 正常的患儿。两组患儿在血清钙、磷以及糖化血红蛋白方面没有显著性差异。有糖尿病肾病的患儿在维生素 D、糖化血红蛋白以及骨密度方面与没有糖尿病肾病的患儿没有显著性差异,但有糖尿病肾病的患儿其血清胆固醇以及三酰甘油显著增高。结论:25羟维生素 D 异常与儿童1型糖尿病存在一定相关性,维生素 D 不足与1型糖尿病患儿骨密度以及血清镁的下降相关。将近三分之二的患儿其维生素 D 不足,显示在1型糖尿病患儿中监测以及补充维生素 D 具有临床意义。 Objective To explore the status of vitamin D in children with type 1 diabetes mellitus(T1DM),its influence on biochemi-cal and densitometric parameters and the relation to diabetic nephropathy. Method 42 children subjects with T1DM at the age 8 ~ 14 years were enrolled to the study. Serum 25OH vitamin D and others clinic chemistry markers were detected. The bone mineral density(BMD)of participants was measured by Ultrasound BMD detector. Serum 25OH vitamin D of 45 healthy children were detected. Results 27 children (64. 3% )had vitamin D level under 75 nmol/ L,from these 13 subjects had vitamin D level under 50 nmol/ L and 2 subjects under 25 nmol / L. Compared with healthy children,T1DM children had a lower serum 25OH vitamin D. Children with vitamin D deficiency had sig-nificantly lower magnesium concentration and lower BMD compared to diabetics with sufficient vitamin D concentrationNo significant differ-ence was found in calcium,phosphorus or HbA1c. Children with diabetic nephropathy showed no significant difference in vitamin D,HbA1c or BMD compared to the children without nephropathy,but children with nephropathy had significantly higher cholesterol and triacylglycerol concentration. Conclusion There are some correlation between Vitamin D abnormalities and type 1 diabetes mellitus in children,vitamin D deficiency is associated with BMD and serum magnesium. Nearly two thirds of children have insufficient vitamin D concentration what sup-ports the need to monitor and eventually supplement vitamin D in T1DM subjects.
