髋关节置换术中Mallory Ⅲ型股骨假体周围骨折的治疗与分析 被引量:3

Causes analysis and treatment of intraoperative periprosthetic femur fractures (Mallory Ⅲ type) in total hip arthroplasty

摘要 目的 探讨髋关节置换术中Mallory Ⅲ型股骨假体周围骨折的产生原因及处理方法,评价术后近期临床结果.方法 对2010年1月至2013年3月河南省人民医院47例(采用Mallory分型,其中Mallory Ⅰ型骨折36例,Mallory Ⅱ型7例,Mallory Ⅲ型4例)髋关节置换术中出现的股骨假体周围骨折患者的临床资料进行回顾性分析.4例Mallory Ⅲ型骨折中3例使用多条钢丝捆扎,1例更换长柄假体联合钢丝捆扎固定.术后平均随访18(11 ~24)个月,对患者进行X线、疼痛及Harris评分等评估.结果 本研究股骨假体周围骨折47例,发生率为4.2%.Mallory Ⅲ骨折4例,发生率为0.35%.4例患者均获随访,骨折均愈合,未发现假体下沉、松动,Harris评分平均91.5分.结论 Mallory Ⅲ型假体骨折术中正确处理可获得很好术中稳定性和近期临床疗效. Objective To investigate the main causes, precaution and treatment of Mallory Ⅲ type intraoperative perprosthetic femur fractures in THA, and evaluate the clinical and radiographic short-term result.Methods Retrospective study was made on 47 patients with intraoperative periprosthetic femur fractures from January 2010 to March 2013.There were 36 cases of Mallory Ⅰ type,7 cases of Mallory Ⅱ type,and 4 cases of Mallory Ⅲ type in this study.Among 4 cases,which were followed-up 11 to 24 months with an average of 18 months.Harris score, pain intensity, and radiographic assessment were used to evaluate the therapeutic effect.Results Intraoperative femur fracture occurred in 47 cases(4.2%) of 1119 hips.There were 4 Mallory Ⅲ type fractures.All patients were followed up, ranged from 11 to 24 months(mean,18 months).All of the fracture was healed up, and no stems were loosened.The mean of harris score at the last follow-up was 91.5.Conclusions The intraoperative of proximal femoral fracture with right method in total hip arthroplasty can improve the stability of the femoral stem.The clinical and radiographic results is satisfactory up to short-term follow-up.
