大鼠脑出血致肠道黏膜屏障损害的研究

摘要 目的:探讨大鼠脑出血应激状态下对肠道黏膜屏障功能的影响。方法:将30只健康成年雄性SD大鼠随机分为脑出血组与对照组,脑出血组采用Nath改良法注射自体动脉血建立大鼠实验性脑出血模型,对照组实验步骤同脑出血组,但不注射自体动脉血,两组大鼠术后按1、3、6、12、24 h分为5个亚组,各亚组大鼠3只。通过测定各亚组大鼠血浆D-乳酸的浓度和小肠二胺氧化酶(DAO)活性来判定脑出血对肠道黏膜屏障功能的影响。取距回盲部5~8 cm小肠组织,制成光镜标本,观察肠绒毛结构的变化,采用四级评分系统(0~3分)对小肠损伤情况进行判定。结果:光镜下观察脑出血组大鼠第3、6、12小时的小肠黏膜屏障损伤评分明显高于对照组(P〈0.05),脑出血后1 h血浆D-乳酸水平即开始升高,第1、3、6、12小时均高于对照组,两组比较差异有统计学意义(P〈0.05),至24 h后下降至略高于对照组(P〉0.05);脑出血组大鼠小肠DAO活性明显下降,随时间变化持续下降,第6、12、24小时低于对照组,两组比较差异有统计学意义(P〈0.05)。结论:大鼠脑出血应激状态下可导致小肠肠道黏膜屏障损害。 Objective: To study the damage of intestinal mucosal barrier function in cerebral hemorrhage model of rat. Methods: Thirty healthy adult male SD rats were randomly divided into cerebral hemorrhage group and control group. The cerebral hemorrhage group was injected with autologous arterial blood by Nath' s improved method,control group was injected nothing. And then,tow groups were divided into five subgroups according to the time interval of 1,3,6,12,24 h,each subgroups with 3 rats. D-lactate concentration and intestinal diamine oxidase( DAO) activity were detected in each subgroups to evaluate the damage of intestinal mucosal barrier function. 5-8 cm small intestinal tissue away from the ileocecal was cuted down and fixed by 10% formaldehyde solution,quaternary score system was used to evaluate the damage of intestinal barrier function under light microscope. Results: The damaged score of the small intestinal mucosa was higher at the time point of 3,6,12 h in the cerebral hemorrhage group than those of control group( P〈0. 05) usernder the light microscope observation; 1 hour after cerebral hemorrhage,D-lactate level began to increase,compared with the control group,the 1,3,6,12 hours subgroups had statistically differences( P〈0. 05),after 24 hours it was down to the lower level and had no statistically significant compared with the control group( P〈0. 05); DAO activity obviously decreased in cerebral hemorrhage group,continuously decline over time,compared with the control group,the 6,12,24 h subgroups were statistically significant( P〈0. 05). Conclusion: The damge of intestinal mucosal barrier function is exist in rats' cerebral hemorrhage model.
