摘要
目的探讨冠状动脉旁路移植与同期心脏瓣膜置换术临床疗效,为临床治疗提供依据。方法选取行冠状动脉旁路移植与同期心脏瓣膜置换术的患者106例,其中风湿性心脏病23例,心功能不全35例,主动脉瓣狭窄12例,心肌缺血症合并心脏瓣膜病28例,二尖瓣狭窄18例,随机分成对照组和治疗组各53例,对照组进行冠状动脉旁路移植手术,治疗组进行冠状动脉旁路移植与同期心脏瓣膜置换术,通过比较两组结果判断疗效。结果两组经过精心术后护理比较,总有效率(CR+PR)比较,差异有统计学意义(x^2=6.135,*=0.015<0.05)。结论应用冠状动脉旁路移植与同期心脏瓣膜置换术治疗心脏瓣膜病合并冠心病,不仅可以提高治疗效果,改善心肌缺血、缺氧状态,而且可以改善和提高患者的生活质量,值得临床推广应用。
Objective To discuss the effect of simultaneous operation of coronary artery bypass grafting and cardiac valve replacement surgery and provide the basis for clinical treatment.Methods 106 patients,23 with rheumatic heart disease,35 with heart failure 35,12 with aortic stenosis,28 with myocardial ischemia with cardiac valvular disease hyperlipidemia,18 with mitral stenosis,who were undergoing simultaneous operation of coronary artery bypass grafting and cardiac valve replacement surgery,were randomly divided into control group and treatment group.53 patients in the control group were given coronary artery bypass graft surgery,while 53 patients in the treatment group received simultaneous operation of coronary artery bypass grafting and cardiac valve replacement surgery.Therapeutic evaluation was conducted by comparing the two results of the two groups.Results After careful comparison of the two groups after treatment,the total efficiency（CR＋PR） chi-square test,explain differences were statistically significant（x^2=6.135,~＊P=0.015〈0.05）.Conclusion Application of coronary artery bypass grafting and cardiac valve replacement surgery earlier treatment of heart valve disease and coronary heart disease,not only can improve the therapeutic effect,improving myocardial ischemia,hypoxia,and can improve and enhance the quality of life of patients,worthy of clinical application.
