伴骨髓水肿的膝骨关节炎与血管内皮因子、缺氧诱导因子1α的相关性研究 被引量:8

Study on the correlation of vascular endothelial growth factor, hypoxia inducible factor-1α and bone marrow edema associated with osteoarthritis of the knee

摘要 目的探讨血清血管内皮因子(VEGF)、缺氧诱导因子1α(HIF-1α)与伴有骨髓水肿的膝骨关节炎的相关性。方法收集我院绝经后女性膝骨关节炎患者50例,根据MRI表现分为骨髓水肿组(25例)和非骨髓水肿组(25例),分别收集其临床症状,WOMAC评分,并同期检测其血清VEGF、HIF-1α水平,采用方差分析比较两组之间的差异以及Spearman相关分析探讨其检测指标水平与骨髓水肿发生程度及其临床指标的相关性。结果 (1)KOA患者血清VEGF、HIF-1α表达水平均呈正相关(r=0.976,P=0.000);(2)与非骨髓水肿组比较,骨髓水肿组血清HIF-1α、VEGF水平更高(F=6.253,P=0.016;F=7.218,P=0.010),夜间痛更明显(F=45.332,P=0.000);(3)骨髓水肿组内,骨髓水肿的容积积分、程度积分分别与血清HIF-1α(r=0.597,P=0.002;r=0.475,P=0.017)、VEGF(r=0.582,P=0.002;r=0.486,P=0.014),夜间痛VAS评分(r=0.456,P=0.022;r=0.732,P=0.000)呈正相关。结论膝骨关节炎伴骨髓水肿时夜间痛明显,血清HIF-1α、VEGF高表达。 Objective To investigate the correlation of the serum and synovial fluid vascular endothelial growth factor (VEGF), hypoxia-inducible factor-lu (HIF-1α) and bone marrow edema associated with osteoarthritis of the knee (KOA). Methods The clinical data of 50 KOA of postmenopansal women, according to MRI, bone marrow edema group (25 cases) and bone marrow edema group (25 cases), were collected. The clinical symptoms, WOMAC score, and the serum VEGF, HIF-1α levels were compared using ANOVA analysis and Spearman correlation analysis. Results (1)The serum VEGF, HIF-1α in osteoarthritis was positively correlated (r=0.976, P=-0.000); (2)the serum HIF-la (F=6.253, P=0.016), VEGF (F=7.218, P=-0.010) levels and hypnalgia (F=45.332, P=0.000) of osteoarthritis with bone marrow edema group was significantly higher than that of the osteoarthritis without bone marrow edema group; (3)the serum HIF-1α, VEGF levels and hypnalgia were associated with osteoarthritis with bone marrow edema in volume (r=0.597, P=-0.002; r=0.582, P=0.002; r=0.456, P=-0.022) and degree (r=0.475, P=0.017; r=0.486, P=014; r=0.732, P=0.000) of integration. Conclusion The pain of the knee joint was obvious in the night with the bone marrow edema, and the HIF-1α, VEGF were high expression.
