比较X线、薄层CT及三维重建检查在急性膝关节损伤诊断中的应用价值 被引量:6

Comparison of the Application Value Between X-ray, Thin Layer CT and 3D Reconstruction in the Diagnosis of Acute Knee Joint Injury

摘要 目的探讨X线、薄层CT及三维重建检查在急性膝关节损伤诊断过程中的临床价值。方法收集100例急性膝关节损伤患者,所有患者均需要接受X线、薄层CT及三维重建的相关检查,在检查完成后将螺旋CT得到的相关容积型数据传入到后处理工作站中进行处理。结果在正确指数方面,薄层CT的正确指数(0.82±0.03)以及三维重建方式的正确指数(0.81±0.02)明显优于X线诊断的正确指数(0.42±0.01),差异有统计学意义(P<0.05)。但薄层CT的正确指数(0.82±0.03)和三维重建之间的正确指数(0.81±0.02)差异无统计学意义(P>0.05)。薄层CT的准确度84%以及三维重建的准确度80%明显优于X线诊断的准确度62%,但薄层CT和三维重建之间的准确度差异无统计学意义(P>0.05)。结论对于薄层CT以及三维重建检查方式而言,能够起到较好的显示效果,显示图像逼真,对于制定治疗方案有着重要意义,也能够较好的避免手术创伤以及失误,在临床上值得推广应用。 Objective To explore the clinical value of X-ray, thin layer CT and 3D reconstruction in the diagnosis of acute knee joint injury. Methods 100 cases with acute knee joint injury were selected. All the patients underwent X-ray, thin layer CT and 3D reconstruction. After the examination, the relative volume data from spiral CT were treated in post-processing workstation. Re-sults The Youden's index of thin layer CT was (0.82±0.03), and that of 3D reconstruction was (0.81±0.02), much better than that (0.42±0.01) of X-ray(P〈0.05). There was no statistically significant difference in Youden's index between thin layer CT and 3D re-construction (P〉0.05). The degree of diagnostic accuracy of thin layer CT was 84%, and that of 3D reconstruction was 80%, much better than that (62%) of X-ray. But the difference in degree of diagnostic accuracy between thin layer CT and 3D reconstruction was not statistically significant. Conclusion Both thin layer CT and 3D reconstruction are of great significance in formulating the treatment regimen and avoiding operation wound and surgical error for they can well and clearly show the images, so they are wor-thy of clinical application and promotion.
