不同类型脂肪酸饮食对肠促胰岛素及胰岛功能的影响 被引量:1

摘要 目的通过给予糖耐量正常受试者不同类型脂肪酸饮食,探讨其对肠促胰岛素(incretin)、胰岛β细胞功能的影响。方法采用自身对照研究,15例健康受试者经标准饮食洗脱1周后,接受高饱和脂肪酸饮食(HSF)、高不饱和脂肪酸饮食(HMF)之一作为早餐摄入,然后再采用标准饮食洗脱1周,重复试验过程,分别进食两种不同的营养餐。测定进餐后0、30、60、120、180、240 min血糖、血脂、游离脂肪酸、胰岛素、抑胃多肽(gastric inhibitory polypeptide,GIP)和胰高血糖素样肽-1(glucagon-like peptide-1,GLP-1)水平。评估不同类型脂肪酸对肠促胰岛素、胰岛β细胞功能的影响。结果曲线下面积(AUC)分析,进餐后HSF组、HMF组血糖水平无明显改变,两组间差异无统计学意义(P>0.05)。HSF组与HMF组相比,AUCins240min、AUCTG240min和AUCFFA240min均增高,差异具有统计学意义(P<0.05)。进餐后,HSF组与HMF组相比,AUCGIP30min、AUCGIP240min、AUCGLP-1 30min和AUCGLP-1 240min均减低,差异有统计学意义(P<0.05)。胰岛β细胞功能指数分析,两组间ΔI30/ΔG30差异无统计学意义(P>0.05)。HSF组与HMF组相比,AUCI/AUCG增高,差异有统计学意义(P<0.05)。结论健康青年人群中不同类型脂肪酸饮食可影响GIP、GLP-1分泌及胰岛功能变化。此外,高不饱和脂肪酸饮食促进GIP、GLP-1分泌作用最明显,但高饱和脂肪酸饮食可促进更多胰岛素的分泌,胰岛β细胞功能指数也明显增高。 Objective To explore the effect of dietary fat acids on incretin and islet function in healthy adults. Methods Before each test, healthy subjects received a 1-week pre-experiment eucaloric diet. Fifteen subjects consumed two meals containing different fat acids, including high saturated fat acid (HSF) and high monounsaturated fat acid (HMF). On two separate occasions,they underwent a minimum of 1-week washout between meals. At 0,30,60,120,180 and 240 min following meal intake, the plasma concentrations of gastric inhibitory polypeptide( GIP), glucagon-like peptide-1 ( GLP-1 ) and serum concentrations of glucose, insulin, triglycerides (TG) and free fatty acid (FFA) were measured. Results Postprandial glucose did not increase significantly following HSF and HMF meals (P 〉 0.05 ). Compared with HMF meal, significant increase in AUCins240min ,AUCTG2a0min and AUCrFA24ominwas observed following HSF meal (P 〈 0.05 ). Compared with HMF meal, a significant decrease in AUCGi^Omin, AUCG[P240mln, AUCGLp.130rain and AUCGLp.l 240min was found following HSF meal (P 〈0.05). No significant difference was observed inAI30/AG30following HSF and HMF meals (P 〉 0.05). AUC1/AUCc was significantly lower following HMF meal as compared with HSF meal (P 〈 0.05). Conclusion This study demonstrates that the function of GIP, GLP-1 and 13 cell is affected by the dietary fat acids in healthy adults. The HMF meal may stimulate GIP and GLP-1 secretion to a greater extent than HSF meal.
