摘要
目的评价早期肠内营养支持联合大黄灌肠对重症急性胰腺炎的临床治疗效果。方法采用随机分组方法将48例重症急性胰腺炎患者分为试验组和对照组,每组各24例。试验组在常规治疗重症急性胰腺炎的基础上,给予早期肠内营养支持并联合大黄灌肠治疗;对照组在常规治疗重症急性胰腺炎的基础上,给予早期肠内营养支持。结果试验组与对照组比较,试验组腹胀时间和腹痛缓解时间均优于对照组(P<0.05),胃肠道功能恢复时间试验组和对照组无显著性差异(P>0.05),治疗5天后,试验组外周血血清淀粉酶下降的幅度显著大于对照组(P<0.05),试验组和对照组肿瘤坏死因子(TNF-α)无显著性差异(P>0.05),治疗5天、9天后,试验组高敏C反应蛋白(CRP)下降的幅度显著大于对照组(P<0.05)。结论早期肠内营养支持联合大黄灌肠治疗重症急性胰腺炎,对降低血清淀粉酶及血浆TNF-α、CRP水平,缓解患者的腹痛腹胀症状,恢复胃肠道功能,具有积极的治疗作用。
OBJECTIVE To evaluate the clinical effect of early enteral nutrition combined with rhubarb enema in the treatment of severe acute pancreatitis.METHODS 48 cases of severe acute pancreatitis were divided into experimental group and control group with24 cases in each group.On the basis of conventional treatment of severe acute pancreatitis,the experimental group was given early enteral nutrition combined with rhubarb enema treatment;the control group was treated with early enteral nutrition therapy on the basis of conventional treatment of severe acute pancreatitis.RESULTS Compared with the control group,test group time and bloating,abdominal pain relief time were better than the control group（P〈0.05）,gastrointestinal function recovery time of the test group and the control group had no obvious statistical difference（P〉0.05）,5 days after the treatment,experimental group peripheral blood serum amylase decreased the amplitude of greater than in the control group（P〈0.05）,test group and the control group of tumor necrosis factor had（TNF alpha） no obvious statistical difference（P〉0.05）,the treatment of 5 days,9 days after the test groups of high sensitive C reactive protein（CRP） decreased the amplitude greater than control group（P〈0.05）.Conclusion early enteral nutrition combined with rhubarb enema has a positive therapeutic effect in the treatment of severe acute pancreatitis,the reduction of blood amylase and plasma TNF-a,CRP levels,relieve patients with abdominal pain,abdominal distension,recovery of gastrointestinal function.
