摘要
目的:分析肿瘤患者手术切口感染病原菌的分布及耐药性,为临床合理使用抗生素提供依据。方法:回顾性分析2005年1月至2014年6月接受手术治疗的肿瘤患者发生切口感染的标本537例,病原菌的培养采用法国生物梅里埃公司VITEK-2 Compact全自动细菌鉴定药敏分析系统及其相应的配套试剂进行病原菌的鉴定以及药敏试验,分析其病原菌的分布及耐药率,采用SPSS 17.0软件进行分析。结果:肿瘤患者手术感染病原菌中排名前3位的分别为大肠埃希菌81株占15.08%、铜绿假单胞菌77株占14.34%、金黄色葡萄球菌69株占12.85%。大肠埃希菌对氨曲南耐药率最高为72.84%、对亚胺培南耐药率最低为0;铜绿假单胞菌对氨苄西林/舒巴坦耐药率最高为79.22%、对亚胺培南耐药率最低为5.19%;金黄色葡萄球菌对克林霉素耐药率最高为76.81%、而对万古霉素和利奈唑胺的耐药率均为0。结论:临床要重视肿瘤患者这一特殊人群手术切口病原菌的分布及耐药性的变化,根据药敏结果使用抗生素,减少耐药菌的产生。
Objective: To study the distribution and drug resistance of the pathogenic bacteria in tumor patients.Methods: 415 tumor patients with surgical wound infection were retrospectively evaluated VITEK-32 fully automatic expression and original reagent were adopted for bacterial culture,bacteria identification and antimicrobial susceptibility tests. Results: The top three pathogens of the tumor patients with surgical wound infection were escherichia coli with 81 strains( 15. 08%),pseudomonas aeruginosa with 77 strains( 14. 34%) and staphylococcus aureus with 69strains( 12. 85%). To Aztreonam,the escherichia coli had the highest drug resistance rate which was 72. 84%,to imipenem,the escherichia coli had the the lowest drug resistance rate which was 0. To ampicillin,the pseudomonas aeruginosa had the the highest drug resistance rate which was 79. 22%,to imipenem,the escherichia coli had the the lowest drug resistance rate which was 5. 19%. To clindamycin,the staphylococcus aureus had the the highest drug resistance rate which was 76. 81%,to Vancomycin and imipenem,the staphylococcus aureus had the lowest drug resistance rate which was 0. Conclusion: The clinical should pay attention to the pathogenic bacteria distribution and drug resistance of surgical incision of tumor patients. The use of antibiotics should be based on susceptibility results,to reduce the bacterial resistance.
