血清视黄醇结合蛋白和游离脂肪酸对妊娠期糖尿病的早期预测价值 被引量:2

Early predictive values of serum retinol binding protein and free fatty acid for gestational diabetes mellitus

摘要 目的:检测正常妊娠和妊娠期糖尿病(GDM)孕妇在中期妊娠时视黄醇结合蛋白和游离脂肪酸的血清水平,探讨妊娠妇女血清视黄醇结合蛋白和游离脂肪酸早期预测(GDM)的价值。方法采用病例对照研究,检测并比较88例GDM及88例正常妊娠妇女孕16~20周血清视黄醇结合蛋白和游离脂肪酸浓度水平。结果GDM组血清视黄醇结合蛋白和游离脂肪酸浓度均高于对照组(P〈0.05),血清视黄醇结合蛋白和游离脂肪酸水平呈正相关(r=0.336,P〈0.01)。血清视黄醇结合蛋白≥30.45mg/L时,预测(GDM)的敏感性为63.6%,特异性为75.0%;血清游离脂肪酸≥0.575mmol/L时,预测(GDM)的敏感性为56.8%,特异性为63.6%。结论孕16~20周孕妇血清视黄醇结合蛋白水平对于(GDM)的预测价值高于血清游离脂肪酸。 Objective To detect the levels of serum retinol binding protein (RBP) and free fatty acid (FFA) in normal pregnant women and gestational diabetes mellitus (GDM) women during the second trimester of pregnancy, explore the early predictive values of ser- um RBP and FFA for GDM. Methods A ease-control study was performed to detect and compare the levels of serum RBP and FFA be- tween 88 GDM women ( GDM group) and 88 normal pregnant women ( control group) during 16-20 gestational weeks. Results The levels of serum RBP and FFA in GDM group were statistically significantly higher than those in control group ( P〈0. 05 ), serum RBP level was pos- itively correlated with serum FFA level (r=O. 336, P〈O. O1 ) . When serum RBP level was equal or more than 30. 45 mg/L, the sensitivity and specificity of GDM prediction were 63.6% and 75.0% , respectively; when serum FFA level was equal or more than 0. 575 mmol/L, the sensitivity and specificity of GDM prediction were 56. 8% and 63.6%, respectively. Conclusion The predictive value of serum RBP level for GDM among the pregnant women during 16-20 gestational weeks was higher than that of serum FFA.
