摘要
目的探究盐酸多奈派齐片(安理申)对脑卒中后抑郁(post-stroke depression,PSD)患者认知功能及生活能力恢复的有效性。方法选择我院2011年10月—2013年9月接诊的PSD患者86例,随机分成观察组(43例)和对照组(43例)。对照组接受原发病治疗和功能康复训练,观察组在对照组的基础上给予安理申治疗。分别在治疗前和治疗后8周、16周采用蒙特利尔认知估量表(montreal cognitive Assessment,MoCA)、Fugl-Meyer运动量表(FMA)及Barthel指数(BI)对患者认知、运动功能和生活活动能力进行评价。结果治疗前两组三项指标差异无统计学意义(P>0.05),而治疗后8周,观察组MoCA、FMA及BI评分为(24.5±1.9)分、(31.2±8.3)分和(35.2±7.5)分,对照组为(20.1±1.4)分、(26.3±8.1)分、(31.8±6.3)分,观察组MoCA、FMA及BI评分明显高于对照组(P<0.05);治疗16周后,观察组MoCA、FMA及BI评分为(28.2±1.3)分、(34.1±8.8)分和(36.2±8.5)分,对照组为(22.5±1.2)分、(27.1±8.3)分、(31.6±6.8)分,观察组MoCA、FMA和BI得分均显著高于对照组(P<0.05)。结论安理申治疗PSD患者能有效提升认知功能,并增强自理能力,是治疗PSD的较佳选择之一。
Objective To explore the efficacy of donepezil hydrochloride on the recovery of cognitive function and ability of daily living in patients with post stroke depression（PSD）.Methods Eighty six patients with PSD from October 2011 to September 2013 were selected,and were randomly divided into two groups：The control group（n=43）treated with routine treatment plus functional reha-bilitation training,the treatment group（n=43）treated with donepezil hydrochloride and routine treatment plus functional rehabilitation training.Montreal Cognitive Assessment（MoCA）,Fugl Meyer Motor Scale （FMA）and Barthel Index （BI）were used to evaluate the cognitive function,motor function and ability of daily living before training,at 8 weeks and 16 weeks after treatment.Results After 8 weeks of treatment,the scores of MoCA,FMA and BI in treatment group were higher than that in control group[（24.5± 1.9）vs（20.1± 1.4）,（31.2± 8.3）vs （26.3± 8.1）,（35.2± 7.5）vs （31.8± 6.3）,P〈0.05].After 16 weeks of treatment,the scores of MoCA,FMA and BI in treatment group were higher than that in control group[（28.2± 1.3）vs （22.5± 1.2）,（34.1± 8.8）vs （27.1± 8.3）,（36.2± 8.5）vs （31.6± 6.8）,P〈0.05].Conclusion Donepezil hydrochloride can improve cognitive function and enhance the ability of self care in patients with PSD.
