长期留置尿管对前列腺穿刺阳性率的影响 被引量:1

The effect of long-term indwelling urethral catheter on the positive rate of prostate biopsy

摘要 目的研究长期(〉12周)留置尿管是否影响前列腺穿刺阳性率。方法在接受前列腺穿刺患者中,根据不同前列腺特异性抗原(PSA)水平比较长期留置尿管组与对照组患者前列腺体积、前列腺穿刺阳性率。结果留置尿管组患者40例,对照组196例。留置尿管组患者前列腺平均体积(93.49±49.90)mL,对照组前列腺平均体积(68.61±38.09)mL,两组患者前列腺体积差异具有统计学意义(P=0.007)。4ng/mL〈PSA≤10mg/mL患者中,留置尿管组11例前列腺穿刺结果均为阴性;对照组98例中14例为前列腺癌(14.3%)。PsA〉10ng/mL患者中,留置尿管组与对照组前列腺穿刺阳性率分别为44.8%和58.2%,差异无统计学意义(P=-0.205)。结论如果能够排除其他影响血清PSA因素,当血清PSA〉10ng/mL时,长期(≥2周)留置尿管对前列腺穿刺阳性率没有影响;建议行前列腺穿刺活检除外前列腺癌。 Objective To study the effect of long-term (≥2 weeks) indwelling urethral catheter on the positive rate of prostate biopsy. Methods The difference of prostate volume and the positive rate of prostate biopsy were comparatively analyzed between the patients with or without indwelling urethral catheter who underwent prostate biopsy according to PSA levels. Results There were 40 patients with urethral catheter and 196 patients in the control group. The mean volume of the prostate in the patients with and without catheter was (93.49±49.90)mL and (68.61±38.09)mL respectively, the difference was significant (P=-0.007). Among patients with 4ng/ml〈PSA≤ 10mg/mL, the results of prostate biopsy for all the 11 patients with catheter were negative, and the results of 14 (14.3%) in the control group were positive in prostate biopsy. Among patients with PSA〉I 0ng/mL, the positive rate of prostate biopsy was 44.8% and 58.2% respectively, the difference was not significant (P=0.205). Conclusion If other influence factors can be excluded, long-term (≥2 weeks) indwelling urethral catheter has no influence on the positive rate of prostate biopsy under PSA level over more than 10ng/mL, and patients with PSA 〉 10ng/ml, should accept prostate biopsy.
