摘要
目的:探讨二维斑点追踪显像技术评价急性心力衰竭患者经冻干重组人脑利钠肽(新活素,rh BNP)治疗后左心室收缩功能变化的价值。方法:40例急性心力衰竭患者均给予常规治疗的基础上应用新活素,分别于新活素治疗前及治疗后常规二维超声心动图检查获得左心室舒张末期内径及左心室射血分数(LVEF),然后应用STI技术检测左心室各心肌节段峰值收缩期应变及左室长轴平均总应变,同时设40名健康志愿者为对照组。结果:与对照组比较,急性心力衰竭患者新活素治疗前左心室舒张末期内径增大、LVEF减低、左心室各心肌节段峰值收缩期应变及左室长轴平均总应变绝对值减小(P均<0.01);rh BNP治疗后患者左心室舒张末期内径较治疗前减小、LVEF较治疗前增加、左心室各心肌节段峰值收缩期应变及左室长轴平均总应变绝对值较治疗前增加,差异均有统计学意义,P均<0.05。rh BNP治疗前、后左室长轴平均总应变绝对值与LVEF均呈良好相关性。结论:rh BNP治疗能有效改善急性心力衰竭患者左心室收缩功能,二维斑点追踪显像技术无角度依赖性、重复性强、具有良好的时间及空间分辨率,可定量、客观的评价左心功能变化,较常规超声更真实地定量评价局部心肌运动及更敏感地检测出心肌运动障碍,此为新活素疗效评定和急性心力衰竭患者预后判定提供了一种新方法。
Objective: To investigate the value of left ventricular systolic function in patients with acute heart failure after treatment by rhBNP using two-dimensional speckle tracking imaging. Methods: Forty cases of acute heart failure patients were given conventional therapy, respectively, in the application of the rhBNP before and after treatment. The conventional echear- diographic parameters of left ventricular end-diastolic diameter and left ventricular ejection fraction (LVEF) were obtainted. Then the STI technology was applied to detect each segment of the left ventricular myocardial systolic peak strain and left ventricular long axis of the average total strain absolute value. The above data of another 40 healthy volunteers were mea- sured as control. Results: Compared with control group, patients with acute heart failure before the rhBNP treatment, left ven- tricular end-diastolic diameter increased, LVEF decreased, each segment of the left ventricular myocardium of left ventricular peak systolic strain and left ventricular long axis of the average total strain absolute value decreased with statistically signifi- cant differences(P〈0.01). Compared with the data before treatment, rhBNP after treatment in patients with left ventricular end- diastolic diameter was reduced, LVEF increased earlier, each segment of the left ventricular myocardium of left ventricular peak systolic strain and left ventricular long axis of the average total strain absolute value were increased with significant dif- ferences (P〈O.05). Left ventricular long axis of the average total strain showed good correlation with LVEF before and after treatment. Conclusion: rhBNP treatment can effectively improve the left ventrieular systolic function in patients with acute heart failure. 2D-STI is a kind of angel-independent and repeatable method with high time and spatial resolution.
