摘要
目的:初步探讨重庆开县结核病经济损失。方法:在一个月内对门诊120名患者和20名住院患者,通过问卷调查其医疗费支出及本人和陪同者就医时交通住宿费、误工等。然后拫据全年门诊确诊人数和住院人数推算全年因结核病所致直接、间接经济负担及总的经济负担。结果:120名门诊患者确诊前就医人均支出1 100元,确诊后至治疗结束人均就医支出5 025元;20名住院患者住院前就医人均支付4 070元,住院期间人均需支付10 778元。按每年新发病1 200人、住院175人推算,一年直接经济负担约989.4万元。而间接经济负担达1 083.1万元,合计经济损失达2 072.5万元。结论:开县结核病经济负担较重,其中直接经济负担中因不能及时诊治占20%;间接经济负担中,不能外出务工所致损失最多,约占70%。因此加强结核病防治健康教育,及早诊断治疗,对恢复健康、减少经济负担具有重要意义。
Objective To estimate the economic losses due to tuberculosis（TB） in Kaixian County of Chongqing. Methods We selected randomly 120 TB outpatients and collected the direct costs including outpatient cost （free TB medicines excluded） ,transportation ＆ nutrition cost,and indirect costs of the TB patient and caretakers before and af- ter the diagnosis in the last year by questionnaire survey,and selected 20 TB inpatients and collected the same costs by questionnaire survey and medical records. Results The average outpatient cost for TB patients before the diagnosis was about 1 100 yuan and the direct cost after the diagnosis was 5 025 yuan in one year. The average direct costs for TB in- patients before and after the hospitalization were 4 070 and 10 778 yuan,individually. Based on 1 200 new TB patients and 175 TB patients for hospitalization annually, the direct, indirect and total economic burden of TB were 9. 894, 10.831 and 2 072.5 million yuan,individually. Conclusions The economic burden of TB patients is huge in Kaixian County. The patients whose direct burden causes treatment not in time account for 20% of the total patients. The patients who could not go out for work suffered the heaviest indirect losses, accounting for 70% of the total patients. It＇s important to improve health and decrease the economic burden due to TB by health education, especially among patients who go out for work.
