摘要
目的探讨补肾活血化瘀方药联合尼莫地平在治疗轻中度血管性痴呆中的临床应用及其对脂质代谢的影响。方法选取97例经诊断为轻中度血管性痴呆患者,分为治疗组(50例)与对照组(47例),分别给予补肾活血化瘀方药联合尼莫地平治疗及单纯尼莫地平治疗。经12周的治疗后,比较2组患者治疗前后的MMSE评分及ADL评分情况,并对患者进行血脂系列指标的检测,比较TC、TG、Apo E、HDL-ch、LDL-ch在治疗前后的差异,评价2种药物治疗方式的临床疗效及对脂质代谢过程的影响。结果经过12周的药物治疗后,2组患者均出现MMSE评分升高及ADL评分降低,并且治疗组改善幅度较对照组明显增加(P<0.05)。血脂水平检测结果表明,2组患者血脂代谢均有改善,并且治疗组较对照组TC、TG、Apo E、LDL-ch指标均有所降低,而HDL-ch水平升高,差异具有统计学意义。结论补肾活血化瘀方药联合尼莫地平治疗轻中度血管性痴呆能够有效改善患者脂质代谢,对血脂系列指标产生有利的调节作用。
Objective To explore the clinical effects and influence on lipid metabolism of herbs used to tonify kidney ,acti-vate blood circulation and dissipate blood stasis in treating vascular dementia combined with nimodipine .Methods The 97 cases of patients with mild-to-moderate vascular dementia were selected and divided into two groups according to the treat-ment .Patients in treatment group（50 cases） were treated with herbs used to tonify kidney ,activate blood circulation ,dissi-pate blood stasis and nimodipine and patients in control group（47 cases） were treated with nimodipine only .With 12 weeks’ treatment ,MMSE and ADL’s scores and the examination results of serum lipid （TC ,TG ,ApoE ,HDL-ch and LDL-ch ） were compared between two groups to evaluate herbs’ clinical effects and influence on lipid metabolism .Results With 12 weeks’ treatment ,in both two groups ,patients’ MMSE score improves and ADL score decreases and the improvement extent is signif-icantly larger in the treatment group （ P〈0 .05 ） .Serum lipid examination shows that TC ,TG ,ApoE ,LDL-ch level in exper-imental group significantly decreased and HDL-ch level improved when compared with control group .Conclusion Combi-nation of herbs used to tonify kidney ,activate blood circulation and dissipate blood stasis with nimodipine has great clinical effects on treatment of mild-to-moderate vascular dementia and is advantageous in serum lipid metabolism .
