旁流呼气末二氧化碳测量在临床全身麻醉手术中有效性评估 被引量:2

摘要 目的通过与目前中国手术室常用并已在中国上市多年的设备对比,来评估旁流二氧化碳检测模块(M402)在临床全身麻醉手术中应用的有效性。方法深圳大学广东省生物信息检测与超声成像重点实验室研究的一体化旁流呼气末二氧化碳测量技术(M402)设备和同类的二氧化碳产品Drager CAPNOTAT主流呼吸末二氧化碳监护仪(简称Drager主流设备)和Mindray Artema旁流呼吸末二氧化碳监护仪(简称Mindray旁流设备)。选定8例腹腔镜手术全身麻醉插管病人进行监测,记录手术过程中的血气分析值,按以标准条例要求制定的评估方法对该模块在临床麻醉手术中应用的有效性进行评估。结果 2 min分别记录M402设备与Drager主流设备的比对数据为343组,与Mindray旁流设备比对的数据为386组。M402设备与Drager主流设备、Mindray旁流设备呼气末二氧化碳浓度(Et CO2)变化趋势相同,M402设备的Et CO2的标准差分别为0.114 k Pa(0.86 mm Hg)、0.130 k Pa(0.98 mm Hg),最大偏差分别为0.399 k Pa(3.00 mm Hg)、0.266 k Pa(2.00 mm Hg),相关系数分别为0.987、0.980。M402设备的气道呼吸率数据与比对设备的数据基本完全一致,其各组标准差为0.05和0.31,相关系数分别为0.96、0.99。M402设备临床使用的稳定性与准确性得到了验证,并符合95%置信区间的一致性要求。M402设备与对照Drager主流设备、Mindray旁流设备之间具有良好的一致性。结论被测二氧化碳模块M402在麻醉手术中可以作为指导医生的客观参数之一。 Objective To evaluate the clinical application effectiveness of side-stream end-tidal CO2(Et CO2) measurement module(M402) in anesthesia surgery by comparing the Et CO2 equipments that commonly used in domestic market for many years.Methods The side-stream Et CO2 equipment M402 and congeneric CO2products(Drager Mainstream and Mindray Sidestream equipment) were used to monitor 8 cases of general anesthesia patients, while the value of blood gas analysis were recorded during the surgery. Statistical methods were used to analyze clinical data, and evaluated effectiveness of module in clinical anesthesia surgery for standard regulations. Results 2-minute data from M402 and Drager were recorded and divided into 343 groups, and the data for Mindray comparing were 386 groups. The changes of end-tidal CO2(Et CO2) in M402 and that of Drager, Mindray were as the same pace, the standard deviations of Et CO2 in M402 were 0.114 k Pa(0.86 mm Hg) and 0.130 k Pa(0.98 mm Hg), the maximum deviations were 0.399 k Pa(3.00 mm Hg) and 0.266 k Pa(2.00 mm Hg), and correlation coefficients were 0.987 and 0.980. The data of Aw RR in M402 was substantially consistent, the standard deviation of each group was 0.05 and 0.31, and correlation coefficients were 0.96 and 0.99. The stability and accuracy of M402 were verified, which met the conformance requirements of 95 % confidence interval, and M402 and had good consistency with control group of Drager and Mindray. Conclusion It is demonstrated that the Et CO2 module M402 could be used as one of objective parameters to provide guidance for clinical use.
