摘要
目的探讨不同基础疾病ICU患者感染血中尿酸的水平高低及其与患者预后程度的相关性。方法根据疗效将ICU 256例感染患者分为死亡组82例、好转组128例及治愈组46例,采用酶测定法测量患者入院24h内肌酐、尿素氮和尿酸值;比较不同基础性疾病患者的血尿酸水平及3组患者的肌酐、尿酸、尿素氮值。结果治愈组患者的尿酸、肌酐、尿素氮值均低于死亡组,差异有统计学意义(P<0.05);患有高血压、脑中风、冠心病、体外损伤、肝硬化、中枢神经系统感染、尿路感染等基础疾病的患者尿酸水平较无基础疾病患者高,差异有统计学意义(P<0.05);血尿酸水平与体内肌酐、尿素氮的值有正相关性,组间比较差异有统计学意义(P<0.05)。结论 ICU感染患者的血清尿酸水平与预后存在一定的相关性,一定程度上能评估患者的病情及预后程度。
OBJECTIVE To explore the correlation between the serum uric acid level of the ICU patients with different underlying diseases complicated with infections and the prognosis.METHODS A total of 256 ICU patients with infections were divided into the death group with 82 cases,the improvement group with 128 cases,and the cure group with 46 cases according to the efficacy.The values of creatinine,urea nitrogen,and uric acid were measured by using enzyme determination method within 24 hours after the admission to the hospital.The serum uric acid levels were compared among the patients with different underlying diseases,and the values of creatinine,urea nitrogen,and uric acid were compared among the three groups of patients.RESULTS The values of uric acid,creatinine,and urea nitrogen of the cure group were lower than those of the death group,and the difference was significant(P〈0.05).The uric acid levels of the patients with hypertension,stroke,coronary heart disease,injury,liver cirrhosis,central nervous system infections,and urinary tract infections were significently higher than those of the patients without those underlying diseases(P〈0.05).The serum uric acid level was positively correlated with the values of creatinine and urea nitrogen,and there was significant different between groups(P 0.05).CONCLUSION The serum uric acid level of the ICU patients with infections is,to a certain degree,associated with the prognosis and is,to some extent,beneficial to the evaluation of illness condition and prognosis of the patients.
