摘要
目的观察使用文蛤膏外敷配合三阶梯药物止痛治疗癌症疼痛（癌痛）的临床疗效。方法选择2013年1月一2014年12月收治的100例癌痛患者作为研究对象，随机分为对照组和治疗组各50例。对照组给予三阶梯药物止痛治疗（第一阶梯：阿司匹林50mg/次，口服，3次／d；第二阶梯：氨酚待因1～2片／次，口服，3次／d；第三阶梯：硫酸吗啡控释片10～30mg，次，口服，1次／d），治疗组在对照组的基础上给予文蛤膏外敷治疗，6h／次，1次，d，连用2周。观察并比较两组治疗效果及爆发痛发作情况。计量资料采用t检验，计数资料采用χ2检验，P〈0．05为差异有统计学意义。结果治疗组缓解轻度、中度癌痛的总有效率分别为94．74％、93．75％，均明显高于对照组的61．90％、58．82％，差异均有统计学意义（均P〈0．05）。对照组平均爆发痛发作次数为（1．83±1．62）次／（人·d）；治疗组为（0．79±0．81）次／（人·d）；两组比较差异有统计学意义（P〈0．05）。结论文蛤膏外敷配合三阶梯止痛药物治疗癌痛的效果比较明显，具有较好的止痛作用，值得在临床上普遍推广使用。
Objective To observe the clinical effect of clam cream external application combined with three-step analgesic drugs in the treatment of cancer pain.Methods 100 cases of cancer pain from January 2013 to December 2014 were selected,random- ly divided into control group and treatment group （n=50）.The patients in control group were given three-step analgesic drugs for treatment （the first step：aspirin 50 mg/time,oral,3 times/d;the second step：paracetamol and codeine 1-2 tablets/time,oral,3 times/d; the third step： morphine sulfate controlled-release tablets 10-30 mg/time,oral,1 time/d）,the patients in treatment group were treat- ed with clam cream extemal application on the basis of the control group.The clinical effect and the situation of pain were record and compared.Measurement data was processed by t test, count data was processed by chi square test,P〈0.05 was considered sta- tistically significant.Results The response rate to release mild and medium cancer pain of treatment group were 94.74%, 93.75%,higher than that of the control group （61.90%,58.82%）,differences were statistically significant（all P〈0.05）.The average number of pain in treatment group was （1.83 ± 1.62）count/（people. d）,in control group was （0.79 ± 0.81）count/（people· d）,difference was statistically significant（P〈0.05）.Conclusion Clam cream external application combined with three-step analgesic drugs in the treatment of cancer pain is more noticeable effective,has a better analgesic effect,it is worthy of clinical application.
