七氟烷复合瑞芬太尼在小儿唇腭裂手术中的临床应用 被引量:2

Clinical comparison of sevoflurane and remifentanil anesthesia for children with cleft lip and palate repair surgery

摘要 【目的】在小儿唇腭裂修复术麻醉中对七氟烷与瑞芬太尼联合应用作一研究。【方法】选择3个月至3岁择期行唇腭裂修补术的患儿60例,男女不限,随机法分为2组,每组30例,七氟烷组采用七氟烷和瑞芬太尼麻醉;丙泊酚组采用丙泊酚和瑞芬太尼麻醉。记录2组患儿在麻醉诱导前(T0)、插管后1 min(T1)、手术开始时(T2)、手术结束前(T3)的心率、平均动脉压(MAP)、脉搏血氧饱和度(s PO);记录气管插管拔管时间、清醒时间和不良反应情况。【2结果】丙泊酚组患儿平均动脉压、心率在T1、T2较T0降低,同七氟烷组比较有统计学意义(P<0.05),而在T3时轻度恢复;七氟烷组患儿拔管时间和清醒时间均短于丙泊酚组(P<0.05),七氟烷组躁动和咳呛等不良反应发生率显著低于丙泊酚(P<0.05)。【结论】七氟烷与瑞芬太尼复合用于小儿唇腭裂修补术,麻醉诱导平稳,效果良好,术后躁动的发生率较低,安全性高,值得临床选择应用。 【Objective】To compare the clinical efficacies and safety of sevoflurane with remifentanil anesthesia for children with cleft lip and palate repair surgery.【Methods】Select 60 pediatric patients undergoing cleft lip and palate repair surgery, between 3 months to3 years old. All patients were randomly divided into 2 groups(n=30 each).Group sevoflurane:sevoflurane and remifentanil anesthesia;and group propofol:propofol and remifentanil anesthesia.Heart rate(HR), mean arterial pressure(MAP)and pulse oxygen saturation(s PO2)were recorded at the time before the anesthesia induction(TO), 1 minute after trachea cannula(T1),the beginning of surgery(T2)and the end of surgery(T3), record the trachea cannula extubation time, waking hours, and adverse reactions.【Results】Propofol group with mean arterial pressure, heart rate in T1, T2 is to reduce than T0, compared with sevoflurane group was statistically significant(P〈0.05),while in T3 mild revovery; Sevoflurane group, all children decannulation time and waking hours shorter than the propofol group(P〈0.05), sevoflurane group of agitation and the incidence of adverse reactions such as cough choke significantly lower than that of propofol(P〈0.05).【Conclusions】Sevoflurane and remifentanil are used for children with cleft lip and palate repair, the induction of anesthesia is stable,the effect is good, the incidence of postoperative agitation is low, the safety is high, and it is worthy of clinical application.
