摘要
目的研究血清维生素D水平与糖尿病周围神经病变的相关性。方法收集2型糖尿病住院患者200例,一组为糖尿病不伴有周围神经病变(NDPN)组111例;另一组为糖尿病合并周围神经病变(DPN)组89例。收集所有受试者临床资料,采用化学发光法检测血清25(OH)D水平。结果 DPN组平均血清维生素D(36.99±7.26 nmol/L)低于NDPN组(57.31±10.27nmol/L),84.3%的DPN患者存在维生素D缺乏,59.5%的NDPN患者存在维生素D缺乏,差异有统计学意义(P<0.05)。血清维生素D水平与病程、Hb A1c、TC和LDL-C呈明显负相关(P<0.05)。Logistic回归分析显示,维生素D缺乏(<50 nmol/L)、病程和Hb A1c是糖尿病周围神经病变的独立危险因素。结论维生素D缺乏在糖尿病合并周围神经病变患者中更为普遍,维生素D缺乏与糖尿病周围神经病变的发生相关,补充维生素D是否能延缓或阻止DPN的发生及进展需进一步研究证实。
【Objective】 To evaluate the associations of serum vitamin D with peripheral neuropathy in type 2 diabetes patients. 【Methods】 Totally 200 patients with type 2 diabetes mellitus were enrolled, including 89 subjects with diabetic peripheral neuropathy(DPN) and 111 subjects without DPN(NDPN). Their clinical data were collected. Fasting blood samples were collected for estimation of vitamin D [25(OH)D] by chemiluminescent immunoassay. Other routine biochemical parameters including renal, liver, lipid profile,Hb A1 c, parathyroid hormone and early-morning urine albumin-to-creatinine ratio were also measured.【Results】 The mean serum vitamin D in the DPN group [(36.99 ± 7.26) nmol/L] was significantly lower than that in the NDPN group [(57.31 ± 10.27) nmol/L, P〈0.05] and 84.3% of patients with neuropathy had vitamin D deficiency compared with 59.5% of NDPN patients(P〈0.05). Serum vitamin D showed significantly negative correlations with duration of diabetes, Hb A1 c, TC and LDL-C(P〈0.05). Logistic regression analysis showed that vitamin D deficiency(〈 50 nmol/L), duration of diabetes and Hb A1 c were the independent risk factors of DPN. 【Conclusions】 Vitamin D deficiency is more commonly present in diabetic patients with peripheral neuropathy than those without. It is closely associated with the occurrence of diabetic peripheral neuropathy. Further studies are required to figure out if vitamin D supplementation could prevent or delay the onset.
