摘要
目的探讨血清细胞角蛋白19片段(CYFRA21-1)、神经元特异性烯醇化酶(NSE)及胃泌素释放肽前体(Pro GRP)对诊断原发性肺癌的价值。方法临床纳入150例肺癌病例、50例良性肺病病例、80例健康体检者,所有受检者均检测血清CYFRA21-1、NSE和Pro GRP水平,分析比较各组的CYFRA21-1、NSE和Pro GRP的阳性率。结果肺癌患者血清CYFRA21-1、NSE和Pro GRP检测阳性率为43.33%、41.33%、10.67%,明显高于良性肺病患者和健康体检者,良性肺病患者阳性率高于健康体检者(P<0.05或P<0.01);肺癌患者、良性肺病患者、健康体检者3种标志物血清水平差异有统计学意义(P<0.01),依次为肺癌患者>良性肺病患者>健康体检者;肺癌不同病理类型组间3种标志物水平差异有统计学意义(P<0.01),表现为随肺癌临床分期的升高而上升。结论肺癌患者血清CYFRA21-1、NSE和Pro GRP的阳性率及水平均明显升高,且临床分期越高,标志物水平越高,这3种标志物对诊断原发性肺癌具有较高的价值,
Objective To investigate the value of serum cytokeratin 19 fragment （CYFRA21-1）, neurone specific enolase （NSE）, and progastrin releasing peptide （ProGRP） in the diagnosis of primary lung cancer. Methods The subjects included 150 lung cancer cases, 50 cases of benign pulmonary disease, and 80 cases of health examination. All subjects received the detection of serum CYFRA21-1, NSE, and ProGRP levels. Comparison and analysis were made in the positive detection rates of CYFRA21-1, NSE, and ProGRP in each group. Results The positive detection rates of serum CYFRA21-1, NSE, and ProGRP in lung cancer patients were 43.33%, 41.33%, and 10.67%, respectively, which were significantly higher than those in patients with benign lung diseases and those taking the health examination. The positive rates of the patients with benign lung diseases were higher than those of the people taking the health examination（P 〈 0.05 or P 〈 0.01）. There were significant differences in the serum levels of the three markers among the three groups（P 〈0.01）. The levels from the highest to the lowest were in the lung cancer group, benign lung diseases group, and the health examination group, respectively. The difference in the levels of the three markers among the subgroups with different pathological types of the lung cancer group was not significant （P 〈 0.01）. The levels went up along with the increase of the lung cancer clinical stages. Conclusion The positive rates and levels of serum CYFRA21-1, NSE, and ProGRP in lung cancer patients all have increased significantly. The higher the clinical stage is, the higher the levels of the markers are. The three markers have great value in the diagnosis of primary lung cancer.
