摘要
目的:观察健脾益肺汤剂对慢性阻塞性肺疾病急性加重期脾肺气虚型患者生活质量、免疫功能及炎症介质表达的影响,探讨其疗效可能的作用机理。方法:将纳入观察的100例患者随机分为观察组和对照组各50例,对照组给予西医常规治疗;观察组在西医常规治疗的基础上联合应用中医健脾益肺汤剂。观察两组患者治疗前后BODE指数、外周血T淋巴细胞亚群、细胞炎症因子的变化情况。结果:治疗4周后,观察组BODE指数,外周血CD4+、CD8+、CD4+/CD8+淋巴细胞,血清TNF-α、IL-8、IL-10水平均有明显改善,与治疗前比较差异显著(P<0.05);且与对照组比较,改善更为明显。结论:健脾益肺汤联合西药治疗能够更好通过免疫功能降低炎症反应,更好地缓解脾肺气虚型AECOPD患者的病情,改善生活质量,提高临床疗效。
Objective：To observe the effects of Jianpi Yifei Decoction on quality of life,immune function and inflammatory mediators expression in patients with acute exacerbation of chronic obstructive pulmonary disease with Qi deficiency of spleen and lung,and to explore the mechanism of the effect may. Methods：The 100 cases of patients were randomly divided into observation group and control group with 50 cases in each group. The control group was given routine treatment of western medicine;the observation group based on routine treatment of western medicine combined with Jianpi Yifei Decoction. To observe the changes of BODE index,T cell subsets and inflammatory factors in peripheral blood of the two groups before and after treatment. Results：After 4 weeks of treatment,observation of groups of BODE index,peripheral blood CD4＋,CD8＋,CD4＋/CD8＋ lymphocytes,serum TNF,IL-8 and IL-10 levels were significantly improved,before treatment with significant difference（P〈0.05）;and group compared with the control,improve more obvious. Conclusion：Jianpi Yifei Decoction combined with western medicine treatment can better reduce the inflammatory reaction through immune function,better to ease chronic obstructive pulmonary disease with Qi deficiency of spleen and lung,improve the quality of life and improve the clinical effect.
