487例不孕女性甲状腺功能筛查结果分析 被引量:7

摘要 目的:探讨不孕女性甲状腺功能异常的发生情况。方法回顾性分析2014年1月~12月在生殖医学中心拟行IVF治疗的487例不孕患者,根据患者不孕原因分为生育能力正常组、PCOS组、输卵管性不孕组;比较三组女性的月经、生育情况;以及早卵泡期性激素、甲状腺功能异常的发生率。结果 PCOS组患者的月经周期天数明显多于另两组,输卵管性不孕组的妊娠次数多于另两组,差异均有统计学意义(P<0.05);PCOS组早卵泡期血清LH、PRL、T水平及TPOAb值较另两组明显升高,差异有统计学意义(P<0.05),而TSH水平较另两组亦升高,但差异无统计学意义(P>0.05);PCOS组女性亚甲减的发生率高于另两组,差异有统计学意义(P<0.05)。结论排卵障碍与甲状腺功能异常是相互影响,互为因果的;对于不孕女性,特别是排卵障碍患者,应重视甲状腺功能的筛查。 Objective To analyze the thyroid function of infertile women.Methods The data of 487 patients undergone in vitro fertilization-embryo transfer/intracytoplasmic sperm injection (IVF-ET/ICSI) from January 2014 to December 2014 in the reproductive medicine center of The First Affi liated Hospital of Soochow University were retrospectively analyzed. The patients were divided into three groups: normal women, PCOS patients, the tubal infertility women. To compare the menstruation and fertility, incidence of sex hormones of early follicular phase and thyroid function.ResultsThe menstruation cycle of PCOS group was much longer than the other two groups, the pregnancy times of the tubal infertility women group was more then the other groups, the differences had statistical signifi cance; serum LH, PRL, T level and TPOAb was signifi cantly higher of PCOS group than the other two groups, the difference had statistical signifi cance. While the difference of TSH level was not statistically signifi cant among the three groups; the incidence of subclinical hypothyroidism of PCOS group was higher than the other two groups.Conclusion Ovulatory dysfunction and thyroid dysfunction interact with each other; and infertile women, especially the patients with ovulation failure, should pay attention to the thyroid function.
