负性情绪刺激对发作间期偏头痛患者Gamma振荡的脑磁图研究 被引量:5

摘要 目的研究负性情绪刺激对发作间期偏头痛患者Gamma频带振荡的影响。方法应用脑磁图仪记录30例发作间期偏头痛患者和30例年龄、性别相匹配的健康对照者在正性、中性、负性面部表情图片刺激诱导的Gamma频段的脑磁信号。采用汉密尔顿焦虑量表(HAMA)及汉密尔顿抑郁量表(HAMD)对偏头痛患者进行评分。结果与正常对照组比较,偏头痛组负性情绪刺激后M100成份时间窗低频和高频Gamma振荡频谱平均功率值均显著降低(t=-2.319,P=0.024;t=-2.610,P=0.012);M170成份时间窗低频和高频Gamma振荡频谱平均功率值也显著降低(t=-2.233,P=0.029;t=-2.299,P=0.025)。偏头痛组发作间期在负性表情刺激后M100成份时间窗低频和高频Gamma振荡频谱平均功率与HAMD评分呈负相关(r=-0.480,P=0.007;r=-0.407,P=0.025);M170成份时间窗低频和高频Gamma振荡频谱平均功率与HAMD评分呈负相关(r=-0.593,P=0.001;r=-0.398,P=0.029)。结论偏头痛患者发作间期处理负性情绪刺激存在脑功能异常,这种脑功能异常可能是偏头痛和情绪因素共同作用的结果。 Objective To investigate the influence of Gamma-oscillation to negative emotional stimuli in patients with interictal migraine. Methods The Gamma-band oscillatory signatures of neuromagnetie activity of magnetoenccphalography were recorded that evoked by positive, neuter and negative facial expressions pictures in 30 patients in interictal migraine and 30 age and gender matched controls. Migraine patients were assessed by Hamilton anxiety scale (HAMA) and Hamilton depression scale (HAMD). Results Compared with normal control group, the averaged spectral power in both low and high Gamma oscillatory responses to negative emotional stimuli in M100 time window of migraine group were significantly decreased (t = -2. 319,P =0. 024;t = -2. 610,P =0. 012) , and the averaged spectral power in both low and high Gamma oscillatory responses to negative emotional stimuli in M170 time window of migraine group were also significantly decreased ( t = - 2. 233, P = 0. 029 ; t = - 2. 299, P = 0. 025 ). The averaged spectral power in both low and high Gamma oscillatory responses to negative emotional stimuli in M100 time window had negative correlation with HAMD score ( r = - 0. 480,P = 0. 007 ;r = - 0. 407,P = 0. 025) , and the averaged spectral power in both low and high Gamma oscillatory responses to negative emotional stimuli in M100 time window had negative correlation with HAMD score (r = -0. 593,P = 0. 001 ;r = -0. 398,P = 0. 029). Conclusion The patients with interictal migraine have brain dysfunction in the processing of negative emotional stimuli, and this brain dysfunction is resulted from both migraine and emotional factors.
