摘要
目的:对比观察高龄硬核白内障小切口非超声乳化与超声乳化术后角膜内皮细胞的变化。方法:回顾性分析行小切口非超声乳化的白内障囊外摘除联合人工晶状体植入术者为A组36例40眼,超声乳化术联合人工晶状体植入术者为B组38例46眼,比较两组手术时间,术前,术后1、6mo裸眼视力、角膜内皮的变化。结果:两组的手术时间比较无差异;术后1mo UCVA≥0.5的术眼,两组间比较差异有统计学意义(χ2=5.174,P=0.023);术后1、6mo两组间角膜内皮细胞计数比较,差异有统计学意义(t=2.565,P=0.038;t=2.774,P=0.041),角膜内皮细胞丢失率比较,差异均有统计学意义(P<0.05)。术后1、6mo两组间细胞平均面积(AVC)比较,差异有统计学意义(t=2.225,P=0.045;t=2.153,P=0.039);两组术后1、6mo两组间变异系数(CV)比较,差异均有统计学意义(P<0.05)。结论:小切口非超声乳化是治疗高龄硬核白内障患者更安全、有效的手术方法。
AIM：To observe and compare the changes of corneal endothelial cells with after phacoemulsification and small incision non- phacoemulsification in elderly hard nuclear cataract. METHODS： In this retrospective analysis, the patients received small incision non-phacoemulsification cataract extraction combined with intraocular lens implantation were as group A, 36 cases （40 eyes） and those underwent ultrasonic phacoemulsification combined with intraocular lens implantation were for group B, 38 cases （46 eyes）. Operation time, preoperative, postoperative land 6too, uncorrected visual acuity, corneal endothelial changes were compared. RESULTS： The operation time of the two groups had no significant difference. UCVA of postoperative 1 mo was ≥ 0.5 after surgery, the difference between the two groups had statistical significance （X2 = 5. 174, P= 0. 023）. One month and 6 mo after operation, the corneal endothelial cell counts of two groups were compared, with statistical significance （ t= 2. 565, P= 0. 038 ; t= 2. 774, P= 0. 041 ） and the difference of corneal endothelial cell loss rate between two groups had statistical significance （P〈0.05）. The difference of AVC between two groups at postoperative 1 and 3too had statistical significance （ t= 2.225, P= 0. 045; t= 2. 153, P= 0. 039）. The coefficient variation of two groups had statistically significant difference （P〈0.05）. CONCLUSION： Small incision non-phacoemulsification operation is a more safe and effective surgery method for treating elderly hardcore cataract patients.
