摘要
目的利用角膜内皮细胞显微镜检测合并高度近视的白内障患者晶状体超声乳化手术前后的角膜内皮细胞,探讨其角膜内皮细胞的变化。方法临床病例对照研究。对2011年11月至2012年9月在上海徐汇区中心医院行晶状体超声乳化手术的白内障患者,根据眼轴童26.0mm为高度近视组,23.0mm至眼轴〈25.0mm作为对照组。在术前及术后3天、1月及3月时分别行常规眼科检查,利用角膜内皮细胞显微镜检测中央角膜厚度(CCT)、内皮细胞密度(CD)、平均内皮细胞面积(AVE)、变异系数(CV)。采用SPSS13.0对数据进行统计学分析。结果共人选高度近视组63例(80只眼)和对照组72例(86只眼),高度近视组眼轴明显长于对照组,但年龄更小(P〈0.01)。高度近视组术前及术后各观察期的CCT、CV与对照组差异无统计学意义(P〉0.05),CD和AVE在术前、术后3天及术后1月与对照组相比差异有统计学意义(P〈0.05)。高度近视组术后各期内皮细胞丢失率和CCT增加率与对照组相比差异无统计学意义(P〉0.05)。组内比较发现高度近视组CCT在术后3月恢复术前水平,而对照组只需1个月,两组术后均CD逐渐减少,AVE逐渐变大,术后1月趋于稳定;高度近视组CV在术后3天内变异明显,而对照组无明显改变。结论高度近视患者白内障术后角膜内皮细胞的损伤和修复过程与非高度近视患者基本相同,但其受损伤敏感性高且损伤后修复所需时间更长。
Objective To explore the changes of corneal endothelial cell after phacoemulsifica tion in cataract patients with high myopia by corneal endothelial microscope. Methods Cataract pa tients underwent phacoemulsification between November 2011 and September 2012 was divided into high myopia group (axial length =〉 26.0rnrn) and control group (23.0 mm 〈 axial length 〈 25.0mm). Routine examinations were done and the parameters of corneal endothelial cell including central corneal thickness (CCT), cell density (CD), average area of endothelial cell (AVE) and coeffi cient of variation (CV) were recorded by corneal endothelial microscope preoperation and at three days, one month and three months postoperation. The data were analyzed by SPSS 13.0 statistical software. Results Sixtythree high myopia patients (80 eyes) and seventytwo control patients (86 eyes) were enrolled. Compared to control group, the axial length of high myopia group was signifi cantly longer and the age was significantly younger (P 〈0.01). There were no significant differences of CCT and CV both preoperation and each observation time postoperation between the two groups (P 〉0.05). CD and AVE preoperation and at three days, one month postoperation in high myopia group were significantly different to those in control group (P 〈0.05). There were no signifi cant differences of the percentage of corneal endothelial cell loss and percentage of increase in CCT neither at each observation time postoperation between the two groups (P 〉0.05). Compared to thedata of preoperation, CCT recovered to the level of preoperation at three months postoperation in high myopia group and at one month postoperation in control group. CD was reduced and AVE were increased gradually until one month postoperation in both groups.
